【摘要】 目的探讨冈上肌出口位摄片在肩关节损伤中的诊断价值。方法分析37例经MRI及手术证实为肩峰撞击综合征和肩袖损伤患者的相关影像学资料,将其影像学诊断结果进行对比。所有患者均摄有患侧肩关节正位,冈上肌出口位,患侧肩关节MRI。根据Bigliani肩峰分型进行分类,同时测量肩峰与肱骨头间距离。结果37例患者中,Ⅰ型肩峰9例,Ⅱ型21例,Ⅲ型7例。出口位x线片测得肩峰下间隙(A—H间距)狭窄31例。其中A—H≤5 mm 7例,5~10 mm 24例。肩峰骨赘20例。肱骨大结节增生21例。结论冈上肌出口位摄片能够提供肩峰分型直接依据,对诊断肩峰撞击综合征和肩袖损伤具有操作简便易行,价格低廉、安全可靠等优点,为临床医师制定诊疗及手术方案提供重要依据。更多还原

【Abstract】 Objective To evaluate the diagnostic value of supraspinatus outlet radiographs inthe shoulder joint injury.Methods Retrospective analysis were performed on 37 cases with subacromial impingement syndrome and rotator cuff injury validated by MRI and surgery. All the affected shoulders were examined by standardanteroposterior radiographs,supraspinatus outlet radiographs,and MRI. The shape of acromionwas classified by the criteria of Bigliani. Also,the distance between the acromion and the humeral head was measured. Results Amongthe 37 patients,there were9 casesof typeⅠ,21 cases of type Ⅱ and 7 cases of type Ⅲ. The supraspinatus outlet radiographs displayedthenarrowed A-H intervalin 31 cases,including7 cases of A-H≤5 mm,24 cases of 5 ~ 10 mm. 20 cases have osteophyte. 21 cases have hyperplasia in the greater tuberosity of humerus. Conclusions The supraspinatus outlet radiographs can provide direct evidence in the classification of acromion. The technique has many advantages,i. e. easy to handle,cheap,safe and reliable,which can play an important role in diagnosing subacromial impingement syndrome and rotator cuff injury.更多还原
