全腔镜食管癌根治术后患者饮食及消化功能的回顾性研究

【摘要】 目的通过对行食管癌根治术的41例患者的病例资料回顾和定期随访结果总结研究,分析全腔镜食管癌根治术后患者的进食及消化功能方面的恢复情况。方法选取行全腔镜手术治疗的41例患者,定期对患者行电话、信件或门诊复诊等方式进行术后饮食状况问卷调查随访,其中确认死亡5例,记录36例患者术后饮食及消化功能的恢复情况。结果有27.78%存活的患者中目前体重与术后出院时相比下降,33.33%体重增加,38.89%体重无明显变化;其中有63.89%诉食欲良好,33.33%食欲一般,2.78%食欲较差;有36.11%已恢复每日三餐,50.00%进食4~5次/d,13.89%患者>5次/d;32.01%的患者日常饮食量恢复正常,39.89%患者日常饮食量一般,27.78%日常饮食量较少;89.89%基本能进食固体食物,其中71.88%患者基本正常饮食固体食物,28.13%患者不能或较少进食固体食物;72.22%进食时及进食后无疼痛及哽咽等不适症,27.78%术后出现过进食时及进食后疼痛或哽咽等不适症;58.33%患者术后没有出现进食后马上有饱腹感症状,41.67%患者术后出现过进食后马上有饱腹感症状;其中47.22%患者诉术后无泛酸、胃部烧灼症状,52.78%患者诉术后出现泛酸、胃部烧灼症状;75.00%患者诉术后无经常腹泻,25.00%诉术后出现经常腹泻症状;16.67%诉进食时嘴发干,5.56%诉术后吻合口狭窄。结论全腔镜食管癌根治术后患者存在一定的并发症,对饮食改变和消化功能造成影响,通过术后规范化的护理可能对于改善全腔镜食管癌恢复及预后提供了可能性。

【Abstract】 Objective To investigate the diet and digestive function of patients after total endoscopic radical resection of esophageal carcinoma minimally invasive esophagectomy. Methods A total of 41 patients with esophageal cancer who underwent total endoscopic radical resection of esophageal carcinoma were enrolled in the study. The postoperative diet status and digestive function recovery status of patients were followed up by questionnaire of telephone,mail and outpatient follow-up.Results The results of follow-up showed that as compared with that before operation,the body weight of 27. 78% of survical patients was decreased,and that of 33. 33% of patients was increased,moreover,more than 38. 89% of patients had no obvious changes. Moreover the appetite in 63. 89% of patients was better,33. 33% of patients was general,2. 78% of patients was loss of appetite. In addition 36. 11% of patients had restored three meals a day,50. 00% of patients had 4 ~ 5 meals a day,13. 89% of patients had more than 5 meals a day. In the aspects of diet uptake,38. 89% of patients was general,28. 13%of patients took little diet,and 88. 89% of patients could take solid diet. In the aspect of pain,27. 78% of patients had pain when taking,and 41. 67% of patients had satiety after meal,52. 78% of patients had pantothenic acid and cauterization in stomach. Besides 25. 00% of patients had diarrhea,75. 00% of patients had no diarrhea,moreover 5. 56% of patients felt anastomotic stenosis and 16. 67% of patients complained dry mouth when taking food. Conclusion There are some complications in patients with esophageal neoplasma after total endoscopic radical resection of esophageal carcinoma,which have effects on diet custom and digestive function of patients,to nurse normatively after operation can improve the prognosis of patients.
