【摘要】 目的观察异维A酸胶丸联合地氯雷他定片治疗痤疮的临床疗效及其对患者外周血TNF-α和IL-8的影响,研究其作用机制。方法将入选的中、重度痤疮80例患者随机分为2组,每组40例。治疗组予地氯雷他定片5 mg口服,1次/d,异维A酸胶丸10 mg口服,2次/d,治疗3个月。对照组予单独口服异维A酸胶丸,10 mg 2/d,2组应用相同的治疗时间。健康组为随机选取的40例健康志愿者。详细记录并评价治疗前中后病情积分及临床疗效,且检测治疗前、后2组在内入选者外周血TNF-α和IL-8的水平。结果治疗12周,病情积分统计对照组(5.01±1.07)分明显高于治疗组(2.13±0.82)分,有效率统计对照组(60.70%)明显低于治疗组(90.07%),对照组治疗后外周血TNF-α、IL-8水平、(17.39±3.41)、(28.7±3.63)pg/ml明显高于治疗组水平(10.42±1.46)、(18.50±2.31)pg/ml,差异均有统计学意义(P<0.05)。病情积分与治疗组IL-8和TNF-α、对照组IL-8、TNF-α水平呈正相关关系。结论异维A酸联合地氯雷他定分散片治疗中、重度痤疮疗效显著,其可能是通过调节患者血清中TNF-α和IL-8的水平发挥治疗作用。更多还原

【Abstract】 Objective To observe the therapeutic effects of desloratadine dispersible tablets combined with low-dose isotretinoin capsule on acne and their effects on serum levels of IL-8 and TNF-α in peripheral blood of patients. Methods Eighty patients with moderate and severe acne were randomly divided into two groups,with 40 cases in each group. The patients in treatment group were treated by taking orally desloratadine dispersible tablets 5 mg,once a day,and isotretinoin soft capsule10 mg,twice a day,for 3 months. However the patients in control group were treated only by taking isotretinoin soft capsules10 mg,twice a day,for 3 months. Moreover 30 volunteers are enrolled as healthy control group. The disease condition scores and clinical curative effects were observed,moreover,the the changes of serum levels of TNF-α and IL-8 before and after treatment were observed and compared among groups. Results After 12-week,the disease condition scores in control group were significantly higher than those in treatment group( 5. 01 ± 1. 07 vs 2. 13 ± 0. 82). The effective rate in treatment group was 90. 07%,which was significantly higher than that( 60. 70%) in control group. Moreover the serum levels of TNF-α[( 10. 42 ± 1. 46) pg/ml and IL-8( 18. 5 ± 2. 31) pg/ml]in treatment group were significantly lower than those [( 17. 39 ±3. 41) and( 28. 7 ± 3. 36) pg/ml,respectively) ] in control group,and these differences were statistically significant( P <0. 05). In addition there was a positive correlation between disease condition scores and levels of IL-8,TNF-α in treatment group as well as between levels IL-8 and levels of TNF-α in control group. Conclusion The desloratadine dispersible tablets combined with isotretinoin capsule has obvious therapeutic effects on acne,its action mechanism may be correlated with regulating the serum levels of TNF-α and IL-8 of patients.更多还原
