【摘要】 目的人附睾蛋白4(HE4)、糖类抗原125(CA125)和ROMA指数和阴道超声检查与上皮性卵巢癌的病理类型与分期的相关性研究。方法选择收治的216例卵巢癌患者,均经影像学、临床及病理等检查方法确诊证实,检测不同的FIGO分期及不同的病理组织类型间上皮性卵巢癌患者HE4、CA125和ROMA指数及阴超检RI、VI的变化情况。结果 HE4、CA125和ROMA指数阳性表达率及表达水平、阴道超声VI随着分期的增加而增加(P<0.05),RI降低(P<0.05)。HE4、CA125阳性表达率及表达水平在浆液性腺癌患者中显著升高,与其他病例类型相比差异有统计学意义(P<0.05);ROMA指数阳性表达率在浆液性腺癌及黏液性腺癌患者中显著升高,与其他病例类型相比差异有统计学意义(P<0.05);不同病理卵巢癌患者RI及VI比较差异无统计学意义(P>0.05)。结论 HE4、CA125和ROMA指数及阴道超声检查与上皮性卵巢癌的病理类型与分期具有一定的相关性;HE4和ROMA能够提高早期上皮性卵巢癌及黏液性腺癌的诊断率,阴道超声能够对卵巢癌上皮性卵巢癌的临床分期有一定的价值,但对病理组织类型的判断无参考意义。更多还原

【Abstract】 Objective To investigate the correlation between HE4,CA125,ROMA index,vaginal ultrasonography and pathological type and staging of epithelial ovarian cance. Methods A total of 216 patients with ovarian cancer were enrolled in the study,who were diagnosed definitely by imaging,clinical and pathological examination. The changes of HE4,CA125,ROMA index,RI,VI in different s FIGO stages of epithelial ovarian cancer were detected and analyzed. Results The expression levels and positive expression rates of HE4,CA125,ROMA index and VI were significantly increased with the increase of staging( P < 0. 05),however RI was significantly decreased( P < 0. 05). The positive expression rates and expression levels of HE4 and CA125 in patients with serous adenocarcinoma were significantly increased,as compared with those in the other pathological types of ovarian cancer( P < 0. 05). However there were no significant differences in RI and VI among different pathological types of ovarian cancer( P > 0. 05). Conclusion The HE4,CA125,ROMA index and vaginal ultrasonography have a certain correlation with the pathological type and staging of epithelial ovarian cancer. HE4 and ROMA can improve the diagnosis rate of early epithelial ovarian cancer and mucinous adenocarcinoma,moreover vaginal ultrasonography is valuable for the clinical stage of epithelial ovarian cancer,but it has no reference value for the judgment of pathological type of ovarian cancer.更多还原
