125I粒子植入治疗前列腺癌的临床研究
Clinical Study of 125I Particle Implantation in Treatment of Patients with Prostate Cancer
【摘要】 目的探讨125I放射粒子植入对Ⅱ期以内前列腺癌的临床效果。方法选取2006年3月—2015年5月绵阳市中心医院泌尿外科收治Ⅱ期以内前列腺癌62例为对象,按照治疗方法分为观察组和对照组,观察组30例,采用经直肠B超引导下经直肠植入125I放射粒子。对照组32例施行前列腺癌根治手术。比较2组手术相关指标、住院天数、满意度及并发症发生率。结果观察组手术时间、术中出血量、术后住院日、输血量及住院费用均少于对照组(P <0. 05),观察组满意度高于对照组,并发症发生率低于对照组(P <0. 05)。观察组1、2、3年生存率均高于对照组(P <0. 05)。2组分别于术后出现2例前列腺特异抗原升高,给予相应治疗后好转。结论采用经直肠径路植入125I粒子治疗Ⅱ期以内前列腺癌疗效满意,且具有创伤小、恢复快、对正常组织损伤低等优势。
【Abstract】 Objective To investigate clinical effects of 125I radioactive seed implantation in treatment of prostate cancer patients within stage II. Methods A total of 62 prostate cancer patients within stage II admitted during March2006 and May 2015 were divided into observation group( n = 30) and control group( n = 32) according to therapeutic methods. Observation group was treated with 125I radioactive seed implantation guided by transrectal type-B ultrasonography,while control group was treated with radical operation for prostate cancer. Operative related indexes,hospitalization expenses,satisfaction degree and incidence rate of complications were observed and compared in two groups. Results In observation group,values of operative time,intraoperative bleeding volume,postoperative hospitalization duration,volume of blood transfusion and hospitalization expenses were significantly lower( P < 0. 05),and satisfaction degree was better,and incidence rate of complications was significartly lower( P < 0. 05),and 1 year,2-years and 3-years survival rates were significantly higher than those in control groups( P < 0. 05). There were 2 patients with increased prostate-specific antigen( PSA) after operation in each group,and symptoms were improved after symptomatic treatment. Conclusion 125I radioactive seed by transrectal implantation in treatment of prostate cancer patients within stage II may achieve good with advantages such as less trauma,quick recovery and less damage to normal tissue.
