【摘要】 目的探讨托吡酯治疗脑梗死后继发癫痫的临床疗效及其对认知功能的影响。方法选择2013年2月—2016年10月海口市人民医院神经内科收治的134例脑梗死后继发癫痫者,按治疗方法分为观察组和对照组,每组67例。观察组给予托吡酯治疗;对照组给予卡马西平治疗。观察两组临床疗效及脑电图异常率,评价神经功能缺损情况、日常生活活动能力和认知功能。结果观察组总有效率高于对照组,脑电图异常率低于对照组(P<0.05)。两组经治疗后神经功能缺损评分低于对照组,日常生活活动能力和认知功能评分高于对照组,且观察组改善程度优于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论托吡酯治疗脑梗死后继发癫痫临床效果较好,可改善脑电图和认知功能。更多还原

【Abstract】 Objective To investigate clinical effectiveness of Topiramate in treatment of secondary epilepsy after cerebral infarction. Methods A total of 134 patients with secondary epilepsy after cerebral infarction admitted during February 2013 and October 2016 were divided into observation group and control group( n = 67 for each group) according to therapeutic methods. Observation group was treated with Topiramate,while control group was treated with Carbamazepine. Clinical effectiveness and abnormal electroencephalogram( EEG) rate were observed,and neurologic impairment conditions,activities of daily living and cognitive function were evaluated in two groups. Results In observation group,the total effective rate was significantly higher,while abnormal EEG rate was lower than those in control group( P < 0. 05). After treatment,scores of National Institutes of Health stroke scale( NIHSS) were significantly lower,while activities of daily living and cognitive function scores in two groups were significantly higher than those in control group,and improved conditions in observation group were superior than those in control group( P < 0. 05). Incidence rate of adverse effects in observation group was significantly lower than that in control group( P < 0. 05). Conclusion Topiramate in treatment of secondary epilepsy after cerebral infarction may achieve good effect and improve EEG and cognitive function.更多还原
