直肠超声引导经会阴前列腺穿刺10+X针法在前列腺癌诊断中的临床应用

【摘要】 目的探讨采用直肠超声引导经会阴前列腺穿刺10+X针法诊断前列腺癌的可行性和安全性。方法选取2017年8月至2018年2月在佛山市第一人民医院超声诊疗中心就诊可疑前列腺癌患者108例,在直肠双平面探头超声引导下经会阴行前列腺穿刺活检,根据病灶实际情况采用10+X针穿刺法,穿刺标本送病理检查,统计穿刺阳性率和并发症发生率。结果病理确诊前列腺癌43例(39.8%),术后出现并发症8例(7.4%),其中血尿4例(3.7%),头晕3例(2.8%),急性尿潴留1例(0.9%)。并发症对症处理后均完全消失,无肠道出血及感染等并发症发生。结论采用直肠超声引导下经会阴前列腺穿刺10+X针法诊断前列腺癌阳性率高,并发症发生率低,简便实用,可以作为诊断前列腺癌的常规方法。

【Abstract】 Objective To explore the feasibility and safety of 10+X needle transperineal prostate biopsy guided by transrectal ultrasound in the diagnosis of prostate cancer. Methods A total of 108 patients with suspected prostate cancer were examined by 10+X needle transperineal prostate biopsy guided by transrectal ultrasound in the First People’s Hospital of Foshan from August 2017 to February 2018. According to the actual condition of the focus, The samples were collected for pathological examination. Positive rate of puncture and incidence of postoperative complication were calculated. Results The pathological diagnosis confirmed prostate cancer in 43 cases(39.8%). Postoperative complications occurred in 8 cases(7.4%), including hematuria in 4 cases(3.7%), dizziness in 3 cases(2.8%), and acute urinary retention in 1 case(0.9%). The complications disappeared completely after symptomatic treatment, and no complications such as intestinal bleeding and infection occurred. Conclusion Transperineal prostate puncture with 10+X needle guided by transrectal ultrasound has a high positive rate in the diagnosis of prostate cancer, a low incidence of complications, and is simple and practical. It can be used as a routine method for the diagnosis of prostate cancer.
