聚乙二醇化重组人粒细胞刺激因子对骨肉瘤化疗患者行预防性中性粒细胞治疗的多中心临床研究

【摘要】 目的观察聚乙二醇化重组人粒细胞刺激因子对骨肉瘤化疗患者进行预防性中性粒细胞恢复治疗中的作用。方法符合入组标准的患者,非随机分成PEG-rh G-CSF预防使用的试验组,和不使用预防生白药物者为对照组,通过监测血常规来观察聚乙二醇化重组人粒细胞刺激因子的对骨肉瘤患者化疗后预防中性粒细胞减少的疗效性和安全性。结果共纳入62例(27例试验组,35例对照组),试验组可显著降低3°~4°中性粒细胞减少的总发生率;对照组1例发生粒细胞缺乏性发热,而试验组无FN发生;监测化验出现中性粒细胞减少后,试验组比对照组应用短效G-CSF的天数总体要短。结论预防性使用聚乙二醇化重组人粒细胞刺激因子可显著降低3°~4°中性粒细胞减少的发生率及缩短3°~4°中性粒细胞减少的持续时间,可降低粒细胞缺乏性发热的发生率,在骨与软组织肉瘤化疗的临床使用中具有良好的安全性和耐受性。

【Abstract】 Objective To observe the effects of pegylated recombinant human granulocyte colony-stimulating factor on supportive neutrophils in patients with osteosarcoma undergoing chemotherapy. Methods The patients who were in accordance with the standard of the group were non-randomly divided into the experimental group with PEGrh G-CSF and the observation grouping. The efficacy and safety of pegylated recombinant human granulocyte colonystimulating factor on neutrophils after prevention and chemotherapy were observed. Results Sixty-two patients( 27 in the test group and 35 in the control group) were included in the clinical observation, and the total incidence of 3-4 degree ANC decreased significantly in the experimental group; the control group had FN in one case, but no FN occurred in the test group. The days of short acting G-CSF in the experimental group were shorter than those in the control group. Conclusions The preventive use of pegylated recombinant human granulocyte colony-stimulating factor can significantly reduce the incidence of 3-4 degree ANC reduction and shorten the duration of 3-4 degree ANC reduction. It can reduce the incidence of FN, and has good safety and tolerance in the clinical use of chemotherapy for patients with osteosarcoma.
