【摘要】 目的探讨早期糖尿病肾病患者N末端前体脑钠肽(NT-pro BNP)水平及其与代谢指标的关系。方法选择85例2型糖尿病患者,依据尿微量白蛋白/肌酐比值分为单纯糖尿病组(SDM组)40例与早期糖尿病肾病组(EDN组)45例;另选取30例健康体检者为正常对照组(NC组)。检测并对比各组的临床资料,分析EDN组中NT-pro BNP水平与代谢指标的关系。结果 EDN组的体质指数、DBP、FBG、餐后2h血糖(2h PBG)、糖化血红蛋白(Hb A1c)、TC、血尿酸(UA)、NT-pro BNP、胱抑素C(Cys-C)水平均高于SDM组及NC组,而SDM组的FBG、2h PBG、Hb A1c、TG、LDL-C、HDL-C水平均高于NC组(均P<0.05)。相关性分析结果显示,在早期糖尿病肾病患者中,NT-pro BNP水平与UA、Cys-C水平呈正相关,与Hb A1c水平呈负相关(均P<0.05)。结论早期糖尿病肾病患者NT-pro BNP水平升高,NT-pro BNP、Cys-C、UA与早期糖尿病肾病的发生发展可能存在相关性。更多还原

【Abstract】 Objective To explore the level of N-terminal pro-brain natriuretic peptide( NT-pro BNP) and its relation with metabolic indices in the patients with early diabetic nephropathy. Methods Eighty-five patients with type 2 diabetes mellitus were selected and were divided into simple diabetic group( SDM group,n = 40) and early diabetic nephropathy group( EDN group,n = 45) according to the urinary microalbumin/creatinine ratio. Another 30 healthy subjects undergoing physical examination were enrolled as normal control group( NC group). The clinical data of all groups were detected and were compared. The correlation of NT-pro BNP level with metabolic indices was analyzed in the EDN group. Results The levels of body mass index,DBP,FBG,2-hour postprandial blood glucose( 2 h PBG),hemoglobin A1 c( Hb A1 c),TC,uric acid( UA),NT-pro BNP and cystatin C( Cys-C) in the EDN group were higher than those in the SDM group and NC group,and the levels of FBG,2 h PBG,Hb A1 c,TG,LDL-C and HDL-C in the SDM group were higher than those in the NC group( all P <0. 05). The results of correlation analysis showed that NT-pro BNP level positively correlated with UA and Cys-C levels but negatively correlated with Hb A1 c level in the patients with early diabetic nephropathy. Conclusion Elevated level of NT-pro BNP is seen in the patients with early diabetic nephropathy,and NT-pro BNP,Cys-C and UA might correlate with the occurrence and progression of early diabetic nephropathy.更多还原
