高血压脑出血血肿周围水肿的危险因素分析

【摘要】 目的分析高血压脑出血血肿周围水肿的危险因素。方法选取本院于2010年3月~2017年12月收治的88例高血压脑出血血肿周围水肿患者(观察组)作为研究对象,选择同期68例高血压脑出血无水肿患者(对照组)。回顾性分析两者临床资料及高血压脑出血血肿周围水肿的危险因素。结果两组周围水肿影响因素分析显示,观察组与对照组的性别、年龄、病程、甘露醇比较,差异无统计学意义(P>0.05);组间氨氯地平、RAS系统降压药物、血压变异性情况比较差异有统计学意义(P<0.05)。通过Logistic回归分析,病程10~20d、氨氯地平、RAS系统降压药物、血压变异性对高血压脑出血周围水肿的影响较大,差异有统计学意义(P<0.05)。结论针对高血压脑出血患者应予以合理地降压药物治疗,注意收缩压控制,便可有效避免周围水肿扩大。

【Abstract】 Objective To explore the risk factors of edema around hematoma in hypertensive cerebral hemorrhage. Methods The clinical data of 88 patients with hypertensive intracerebral hemorrhage and edema around the hematoma in our hospital from March 2010 to December 2017 were retrospectively analyzed and selected as observation group, and 68 patients with hypertensive intracerebral hemorrhage and without edema around hematoma in the same period were selected as control group. The clinical data of two groups were compared and the risk factors of edema around hematoma in hypertensive intracerebral hemorrhage were analyzed. Results The gender, age, disease duration, and mannitol of two groups showed no significant difference(P > 0.05). The level of amlodipine and RAS antihypertensive drugs, comparison of blood pressure variability of two groups showed statistically significant(P < 0.05). After Logistic regression analysis, the duration of disease(10-20 d), amlodipine, RAS antihypertensive drugs, comparison of blood pressure variability had great influence on the edema around hypertensive intracerebral hemorrhage, and the difference was statistically significant(P < 0.05). Conclusion The patients with hypertensive intracerebral hemorrhage should be treated with reasonable antihypertensive drugs and pay attention to contractile pressure control, which can effectively avoid the expansion of peripheral edema.
