妊娠期糖尿病患者PAPP-A、CRP与糖脂代谢的相关性
Correlation between PAPP-A, CRP and glycolipid metabolism in gestational diabetes mellitus
【摘要】 目的探讨妊娠期糖尿病患者妊娠相关蛋白A(PAPP-A)、C反应蛋白(CRP)水平与糖脂代谢的相关性。方法选取2015年10月~2017年4月间诊治的妊娠期糖尿病晚期妊娠孕妇52例设为研究组,另选取健康妊娠晚期孕妇65例为对照组。检测并比较两组血清PAPP-A、CRP、甘油三酯(TG)、总胆固醇(TC)、糖化血红蛋白(Hb A1C)、空腹血糖(FPG)及空腹胰岛素(FINS),并计算胰岛素抵抗指数(HOMA-IR),并探讨PAPP-A、CRP与TG、TC、Hb A1C及HOMA-IR的相关性。结果研究组PAPP-A、CRP、TG、TC、Hb A1C及HOMA-IR水平均高于对照组,差异具有统计学意义(P<0.05);PAPP-A与TG、TC、Hb A1C及HOMAIR呈正相关(r=0.462,0.596,0.674,P<0.05),CRP与TG及TC正相关(r=0.842,0.768,P<0.05);Logistic回归性分析法结果显示,血清PAPP-A是TG、TC、Hb A1C及HOMA-IR升高的独立影响因素(P<0.05),血清CRP是TG、TC升高的独立影响因素(P<0.05)。结论妊娠期糖尿病患者血清PAPP-A、CRP水平明显增高,且糖脂代谢紊乱密切相关。
【Abstract】 Objective To investigate the relationship between pregnancy associated protein A(PAPP-A), C reactive protein(CRP) and glycolipid metabolism in gestational diabetes mellitus. Methods Fifty-two cases of gestational diabetes late pregnancy from October 2015 to April 2017 were selected as the study group. Another 65 healthy pregnant women in the third trimester were selected as control group. The serum levels of PAPP-A, CRP, triglyceride(TG), total cholesterol(TC), glycated hemoglobin(Hb A1 C), fasting blood glucose(FPG) and fasting insulin(FINS) were detected and compared in two groups. The insulin resistance index(HOMA-IR) was calculated, and the correlation between PAPP-A, CRP and TG, TC, Hb A1 C and HOMA-IR were discussed. Results The levels of PAPP-A, CRP, TG, TC, Hb A1 C and HOMA-IR in the study group were all higher than those in the control group, the difference was statistically significant(P < 0.05). PAPP-A was positively correlated with TG, TC, Hb A1 C and HOMA-IR(r=0.462,0.596,0.674, P < 0.05), CRP was positively correlated with TG and TC(r=0.842,0.768, P < 0.05). The results of Logistic regression analysis showed that serum PAPP-A was an independent factor in the increase of TG, TC, Hb A1 C and HOMA-IR(P < 0.05), and serum CRP was an independent factor for the increase of TG and TC(P < 0.05). Conclusion The serum PAPP-A and CRP levels significantly increase in gestational diabetes patients, and which are closely related to the disorder of glucose and lipid metabolism.
