不同心肌损伤生化标志物检验的临床价值分析
Analysis on clinical values of different biochemical markers to examination of myocardial injury
【摘要】 目的分析不同心肌损伤生化标志物检验的临床价值。方法随机选取我院2016年1月~2017年4月收治的心血管疾病患者220例,所有患者均经临床诊断确诊为心肌损伤,对其临床资料进行回顾性分析,对比分析五种常见生化标志物检验心肌损伤灵敏度及CK-MB、Mb、cTnI标志物特异性。结果在五种常见心肌损伤生化标志物灵敏度对比中,其检测灵敏度存在一定不同,其中以pro-BNP的诊断灵敏度为100%,远高于CK-MB、Mb、cTnI和Hcy,相比于其他四种生化标志物检验灵敏度来说差异有统计学意义(P<0.05)。在CK-MB、Mb、cTnI标志物特异性对比中,Mb首次出现时间、峰值时间均明显高于CK-MB和cTnI,组间比较差异有统计学意义(P<0.05),另外,cTnI恢复时间明显低于Mb和CK-MB,经统计学对比分析差异显著,具有统计学意义(P<0.05)。结论五种生化标志物在检验心肌损伤患者中,在其发生、发展过程中均起到重要作用,与治疗效果之间存在较强的关联性,只有在检测过程中有效互补,才能够最大程度上提高临床诊断价值。
【Abstract】 Objective To analyze clinical values of different biochemical markers to examination of myocardial injury. Methods Two hundred and twenty patients with cardiovascular disease were randomly selected in our hospital from January 2016 to April 2017,all patients were diagnosed with a clinical diagnosis of myocardial injury,and the clinical data were retrospectively analyzed.Sensitivity of five common biochemical markers to test myocardial injury and specificity of CK-MB,Mb,cTnI markers were compared and analyzed. Results The sensitivity of five common biochemical markers for myocardial injury was different.The diagnostic sensitivity of pro-BNP was 100%,which was much higher than that of CK-MBN,MbTnI and Hcy.Compared with the other four biochemical markers,the diagnostic sensitivity was significantly higher than that of other four biochemical markers(P < 0.05).In CK-MB,Mb,cTnI markers specific contrast,Mb for the first time,the peak time was significantly higher than that of CK-MB and cTnI,the difference was statistically significant between the groups(P < 0.05),In addition,the recovery time of cTnI was significantly lower than that of Mb and CK-MBs(P < 0.05). Conclusion Five biochemical markers play an important role in the development and progression of myocardial injury.There is a strong correlation between the markers and therapeutic effects.The clinical value of these five biochemical markers could be improved to the greatest extent only when they complement each other during the examination progress.Five biochemical markers play an important role in the detection of myocardial injury in the process of occurrence and development.There is a strong correlation with the therapeutic effect.Only by effectively complementing each other in the detection process can the clinical diagnosis value be maximized.
