女性腰1-4和腰2-4椎体DXA测量结果比较

【摘要】 目的对比分析双能X线骨密度测量仪(DXA)测量腰1-4椎体及腰2-4椎体骨密度值、T-值结果及其随年龄变化的异同点,为临床解释骨密度测量结果提供客观依据。方法收集2013年12月至2014年10月在北京协和医院行DXA骨密度测量的女性受检者4 903例,年龄45~94岁,平均(60.3±9.4)岁,将所有受检者依据年龄分为8组。记录每组腰1-4椎体、腰2-4椎体、股骨颈及全髋的骨密度值、T-值,采用配对t检验比较组间差异,并观察腰椎两部位骨密度值及T-值随年龄变化的特点。结果所有分组中,腰1-4椎体骨密度值及T-值均低于同年龄组腰2-4椎体,差异均有统计学意义(P<0.05);70岁前年龄组腰1-4、腰2-4椎体及全部年龄组股骨颈及全髋骨密度值及T-值均随年龄增加而降低,以50~54岁年龄组为著;75岁后腰椎骨密度值及T-值较前略回升。结论增龄后,腰椎1-4和腰椎2-4骨密度具有相同的变化趋势,但腰椎1-4较腰椎2-4的骨密度及T-值均明显降低。

【Abstract】 Objective To analyse the bone mineral density and T-score at lumbar 1-4 and lumbar 2-4 in different aged women through retrospective data of dual energy X-ray absorptiometry(DXA) and explore more valuable information for the diagnosis of osteoporosis. Methods With DXA,the bone mineral density at lumbar(L1-4,L2-4),femoral neck and hip bones were measured in 4 903 females over 44 years old from December 2013 to October 2014. Subjects were45 to 94 years old with a mean age of(60. 3± 9. 4) years. There were 8 groups based upon the stratifications of 5-year age intervals(> 80-year-olds combined as one group). The bone mineral density and T-score at lumbar(L1-4,L2-4),femoral neck,and total hip were recorded. After recording data,the average bone mineral density and T-score were calculated and their differences were analyzed by T-test. Furthermore,we studied the lumbar bone mineral density characteristics with age. Results In all groups,bone mineral density and T-score at L1-4 were lower than L2-4(P<0. 05). With age,bone mineral density and T-score of lumbar spine were decreased before 70 years. The largest decrease were found in50 to 54 years old group. After 75 years,there were some rebound in lumbar bone mineral density and T-score. Conclusion The bone mineral density of L1-4 and L2-4 show the same change trend during aging,but the bone mineral density and T-score of L1-4 are significantly lower than those of L2-4.
