核素显像联合高频彩超诊断继发性甲状旁腺功能亢进症的临床价值

【摘要】 目的:探讨核素显像联合高频彩超诊断继发性甲状旁腺功能亢进症(SHPT)的临床价值。方法:回顾性分析2012年至2018年间75例(男33例,女42例,平均年龄39. 5岁)行甲状旁腺切除术的慢性肾脏疾病SHPT患者核素显像资料以及高频彩超资料,以术后病理结果为"金标准",计算核素显像、高频彩超各自的灵敏性和准确度以及两者联合检查的灵敏性和准确度。结果:核素显像和高频彩超诊断慢性肾脏疾病SHPT的灵敏性、准确度、假阳性率、假阴性率分别为74. 5%、78. 3%、0%、25. 5%和84. 3%、76. 6%、66. 7%、15. 7%。二者联合诊断的灵敏性、准确性、假阳性率、假阴性率分别为100%、94. 2%、38. 9%、0%。二者联合诊断SHPT的灵敏性和准确性差异有统计学意义(P <0. 05)。结论:核素显像联合高频彩超有助于提高SHPT的检出率,检查的准确度也有明显的提高,能更好地为手术治疗提供依据。

【Abstract】 Objective: To explore the clinical value of radionuclide imaging combined with high frequency color Doppler ultrasonography in the diagnosis of secondary hyperthyroidism( SHPT). Methods: Retrospective analysis was made about the radionuclide imaging data and high frequency color Doppler ultrasound of 75 patients( male 33,female 42,average age 39. 5 years) with chronic kidney disease SHPT who underwent parathyroidectomy from 2012 to 2018 years. The postoperative pathological results were the " gold standard". The sensitivity and accuracy of radionuclide imaging and high frequency color Doppler ultrasonography and The sensitivity and accuracy of the combination of the two for SHPT were calculated. Results: The sensitivity,accuracy,False positive rate and False negative of radionuclide imaging and high frequency color Doppler ultrasonography in the diagnosis of SHPT were 74. 5%,78. 3% 、0% 、25. 5% and 84. 3%,76. 6% 、66. 7% 、15. 7%.The sensitivity,accuracy,False positive rate and False negative of the combined diagnosis of radionuclide imaging and high frequency color Doppler ultrasonography were 93. 6%,88. 3% 、63. 6% 、6. 4%. The difference in sensitivity and accuracy between the two methods in diagnosing SHPT was statistically significant. Conclusion: Radionuclide imaging combined with high frequency color Doppler ultrasound is helpful to improve the detection rate of SHPT.The accuracy of the inspection is also significantly improved. It provides a better basis for surgical treatment.
