加长型PFNA与LPFP内固定治疗股骨粗隆下骨折的疗效比较

【摘要】 目的比较加长型股骨近端防旋髓内钉(PFNA)与股骨近端锁定钢板(LPFP)内固定治疗股骨粗隆下骨折的临床疗效。方法回顾性分析自2014-03—2016-08手术治疗的56例股骨粗隆下骨折,采用加长型PFNA内固定治疗28例(PFNA组),采用LPFP内固定治疗28例(LPFP组)。比较2组切口长度、术中出血量、骨折愈合时间及末次随访时髋关节功能Harris评分。结果 PFNA组切口长度较LPFP组短,且术中出血量少于LPFP组,差异有统计学意义(P<0.05)。PFNA组随访时间为(11.8±3.1)个月,LPFP组随访时间为(12.2±2.9)个月。PFNA组骨折愈合时间短于LPFP组,且髋关节功能Harris评分优良率明显高于LPFP组,差异有统计学意义(P<0.05)。结论加长型PFNA内固定治疗股骨粗隆下骨折有创伤小、骨折愈合快、髋关节功能恢复满意、并发症少等优点,是理想的手术治疗方式。

【Abstract】 Objective To compare the clinical effects of extended proximal femoral nail antirotation(PFNA) and proximal femoral locking plate(LPFP) for the treatment of femoral subtrochanteric fractures. Methods Fifty-six patients with femoral subtrochanteric fractures underwent surgical treatment from March 2014 to August 2016. Twenty-eight patients(PFNA group)underwent extended PFNA internal fixation while 28 patients(LPFP group) underwent extended LPFP internal fixation. The incision length, intraoperative blood loss, fracture healing time and Harris hip function score were compared at the final follow-up. Results The incision length of PFNA group was shorter than that of LPFP group, and the intraoperative blood loss was less than that of LPFP group, the difference was statistically significant(P <0.05). The follow-up time was(11.8 ±3.1)months in PFNA group and(12.2±2.9) months in LPFP group. The healing time of PFNA group was shorter than that of LPFP group, and the excellent and good rate of Harris score of hip joint function was significantly higher than that of LPFP group( P <0.05). Conclusion Extended PFNA internal fixation for treatment of femoral intertrochanteric fractures is the ideal surgical treatment with less trauma, rapid fracture healing, satisfactory hip function recovery, fewer complications.
