【摘要】 目的:探讨腹腔镜全腹膜外疝修补术(totally extraperitoneal,TEP)治疗嵌顿性腹股沟疝的应用价值。方法:回顾分析2014年6月至2017年3月为33例腹股沟嵌顿疝患者行腹腔镜手术的临床资料。结果:30例成功完成TEP,3例中转开腹行小切口下小肠部分切除术+内环口高位缝扎术。手术时间42~140 min,平均(63.4±20.3)min;住院2~11 d,平均(4.5±2.2)d,术后出现血清肿7例(23%),无肠梗阻及补片感染等严重并发症发生。术后随访6~57个月,无慢性疼痛及复发。结论:TEP治疗嵌顿性腹股沟疝是安全、可行的,具有创伤小、并发症少、康复快等优势,但术者需具备腹腔镜疝修补手术经验。更多还原

【Abstract】 Objective: To investigate the clinical value of the laparoscopic totally extraperitoneal prosthesis for the patients with inguinal incarcerated hernia. Methods: Clinical data of 33 patients with inguinal incarcerated hernia who underwent laparoscopic operation in the general surgery department of the Second Hospital of Sanming from Jun. 2014 to Mar. 2017 were retrospectively analyzed. Results: Laparoscopic totally extraperitoneal prosthesis was successfully performed in 30 patients,open partial intestinal resection and internal orifice high ligation by small incision was performed in 3 patients. The average operation time of the laparoscopic totally extraperitoneal prosthesis was( 63. 4 ± 20. 3) min( range,42-140 min) and the mean hospital stay was( 4. 5 ± 2. 2) d( range,2-11 d). 7 patients( 23%) with inguinal incarcerated hernia suffered from seroma after operation,no severe postoperative complications occurred such as intestinal obstruction or patch infection. Follow-up from 6 to 57 months of all the cases demonstrated no chronic pain or recurrence after operation. Conclusions: Laparoscopic totally extraperitoneal prosthesis is safe and feasible in the treatment of the patient with incarcerated inguinal hernia,this procedure has advantages of small trauma,few complications and fast recovery,while it needs to be performed by the doctors who have experience of laparoscopic herniorrhaphy.更多还原
