北京市丰台区7~12月龄儿童10μg乙肝疫苗免疫效果随访
Follow-up study of 10μg hepatitis B vaccine immune effect of 7 to 12 months children in Fengtai District
【摘要】 目的采用队列随访的方法,探讨新生儿按照"0-1-6"免疫程序完成三针乙肝疫苗接种后,抗体阳转率、应答水平以及5年后乙肝疫苗的免疫效果,为儿童乙肝免疫策略的制定提供科学依据。方法按照单纯随机抽样原则,2009年抽取丰台区10家社区卫生服务中心的344名完成"0-1-6"乙肝疫苗接种程序的7~12月龄儿童进行调查问卷和采血检测,5年后对随访到的调查对象再次进行问卷调查和静脉采血。结果2009年调查的344名儿童中乙肝表面抗体阳转344人(100. 00%),2014年随访调查的80名儿童中乙肝表面抗体阳性66人(82. 50%)。2009年儿童乙肝表面抗体滴度(GMC)中位数为1547. 5IU/L,2014年儿童乙肝表面抗体滴度(GMC)中位数为43. 5IU/L,衰减程度差异有统计学意义(P <0. 05)。结论北京市丰台区新生儿完成三针乙肝疫苗接种后抗体阳转率较高,5年后,儿童乙肝表面抗体滴度随着免疫时间的延长而明显下降,但仍维持在较高水平。
【Abstract】 Objective Queue follow-up method was adopted to investigate the antibody positive rate,response level and hepatitis B vaccine immune effect after five years of the newborn(who have been completed the " 0-1-6" three needles hepatitis B vaccination immune process) in order to provide scientific basis for the establishment of hepatitis B immunization strategy. Methods Simple random method was adopted,344 children aged 7 to 12 months were randomly selected(who have been completed the " 0-1-6" three needles hepatitis B vaccination immune process from 10 community health service centers in Fengtai District),questionnaires and blood examination were performed,questionnaires and blood examination were repeated after five years. Results All the 344(100. 00%) children investigated in the survey of 2009 turned positive,among the 80 children investigated in the survey of 2014,66(82. 50%) turned positive. Children’s hepatitis B surface antibody median titer of 2009 was 1 547. 5 IU/L,children’s hepatitis B surface antibody median titer of 2014 was 43. 5 IU/L,the attenuation degree had statistical significance. Conclusion Newborns in Fengtai District,Beijing,had a higher positive rate of antibody after completing three doses of hepatitis B vaccination. Five years later children’s hepatitis B surface antibody titer decreased significantly with prolonged immunization time,but still remained at a high level.
