扩散张量成像(DTI)技术在脑部恶性肿瘤中的应用价值

【摘要】 目的探讨高场磁共振扩散张量成像(DTI)技术在恶性脑肿瘤中的应用价值。方法对5例恶性脑膜瘤、15例脑胶质瘤、30例转移瘤行常规头颅平扫及增强,三维成像(AX3DFSPGR)、扩散张量成像(DTI)扫描,利用表观弥散系数(ADC)值、各向异性分数(FA)值进行后处理重建,测量肿瘤实质区、水肿区和瘤周白质区的白质纤维束走行进行综合分析。结果恶性脑肿瘤周水肿区、瘤周白质纤维束均发生破坏、中断,肿瘤周围白质纤维束的破坏发生率与肿瘤的恶性程度成正比。结论扩散张量成像(DTI)技术在鉴别颅内恶性肿瘤方面有重要应用价值。

【Abstract】 Objective To investigate the value of high field magnetic resonance diffusion tensor imaging(DTI) technique in malignant brain tumors. Methods Five cases of malignant meningiomas, 15 cases of gliomas and 30 cases of metastases were scanned by AX3 DFSPGR and diffusion tensor imaging(DTI). The apparent diffusion coefficient(ADC) Value and anisotropy fraction(FA) were post-processed and reconstructed. The white matter fiber bundles in the tumor parenchyma area, edema area and peritumor white matter area were measured and analyzed. Results Malignant brain tumor edema area, peritumoral white matter fiber bundles were destroyed, interrupted, the incidence of white matter damage around the tumor and the tumor is proportional to the degree of malignancy. Conclusion Diffusion tensor imaging(DTI) has important application value in differentiating intracranial malignant tumors.
