儿童湿疹与胃肠积热及相关因素相关性的病例-对照研究
Association between gastrointestinal heat retention syndrome and other correlative factors with eczema in children: A case control study
【摘要】 目的:探索胃肠积热及饮食等相关因素在儿童湿疹发病中的相关性。方法:采用病例-对照研究设计,选择1~14岁的研究对象分为病例组(患有儿童湿疹)与对照组(无儿童湿疹),应用调查表,调查儿童一般信息和中医症状体征等信息,并使用《胃肠积热诊断量表(儿科部分)》评价研究对象是否存在胃肠积热。采用Logistic回归模型探讨儿童湿疹与胃肠积热及饮食、环境等相关因素的关系。结果:共纳入有效研究对象230例,病例组∶对照组=1∶1.28。(1)病例组儿童40.59%存在胃肠积热,对照组儿童6.20%存在胃肠积热(P<0.001,O R=7.200,95%CI[2.574,20.138])。(2)患儿皮肤病史(P=0.001,O R=36.959,95%CI[4.623,295.491])、性别(P=0.016,OR=2.514,95%CI[1.190,5.309])与儿童湿疹的发病呈正相关。结论:本研究发现胃肠积热与儿童湿疹正相关,患有皮肤病史、性别为女孩的在儿童湿疹发病中不可被忽视。
【Abstract】 Objective: To explore the association between eczema in children with gastrointestinal heat retention syndrome and other related factors such as die, as well to interpret the risk factors in the development and pathogenesis of eczema in children. Methods: A case-control study was carried out in outpatients who aged from 1 to 14 years old, and the participants were recruited into case group(children with eczema) and control group(children without eczema). Questionnaires were used to collect data including general characteristics, clinical symptoms, signs and so on. The gastrointestinal heat retention syndrome was identified by a diagnosis scale specific for children. Logistic regression model was used to explore the association between gastrointestinal heat retention syndrome and other related factors with eczema in children. Results: A total of 230 participants were enrolled in the study(case group∶control=1∶1.28).(1)40.59% of children with eczema were diagnosed as gastrointestinal heat retention syndrome, compared with 6.20% in children without eczema(P<0.001, OR=7.200, 95%CI[2.574, 20.138]).(2)Multivariate analysis showed that history of pediatric dermatosis(P=0.001, OR=36.959, 95%CI[4.623, 295.491]), and gender(P=0.016, OR=2.514, 95%CI[1.190, 5.309]) were positively correlated with eczema in children. Conclusion: The gastrointestinal heat retention syndrome is positively associated with eczema in children. History of pediatric dermatosis and gender issues should not be ignored in the pathogenesis of eczema in children.
