穴区弹性纤维在提插手法“抗炎”效应中的作用
Effects of elastic fiber in lifting and thrusting of needle in acupuncture manipulation on anti-inflammatory
【摘要】 目的:探讨穴区弹性纤维在针刺提插手法效应中的作用。方法:30只雄性家兔随机分为6组:假手术组(N组)、模型组(M组)、针刺无手法组(W组)、针刺手法组(A组)、弹性蛋白酶预处理组(TM组)、弹性蛋白酶预处理+针刺手法组(TA组),每组5只。采用细菌内毒素制作家兔发热模型;W组、A组和TA组针刺"曲池"穴进行治疗,W组只针刺留针不做手法,A组留针期间行提插法,TA组用弹性酶预处理后进行A组同样的针刺治疗。造模后2、4、6h检测家兔血清炎性因子IL-1β、TNF-α和抗炎因子IL-4含量,6h后取材并观察穴区局部皮下组织弹性纤维形态。结果:与N组比较,M组血清IL-1β和TNF-α含量显著升高、IL-4显著降低(P<0.01);与M组比较,W组和A组IL-1β和TNF-α含量显著降低、IL-4显著升高(P<0.01,P<0.05);与W组比较,A组IL-1β和TNF-α含量显著降低,IL-4含量显著升高(P<0.01),与A组比较,TA组血清IL-1β和TNF-α含量显著升高、IL-4显著降低(P<0.01)。较N组,A组曲池穴区皮下组织弹性纤维卷曲度有所增强。结论:针刺可以显著抑制内毒素诱发的炎性反应,针刺手法的应用可以显著提高针刺疗效。穴区弹性纤维可能是传导针刺机械刺激的重要环节。
【Abstract】 Objective: To explore the role of elastic fiber in effect of lifting and thrusting of needle. Methods: Thirty male rabbits were randomly divided into 6 groups: sham operation group(group N), model group(group M), acupuncture without manipulation group(group W), acupuncture manipulation group(group A), elastase pretreatment(group TM) and elastase & acupuncture manipulation group(group TA), 5 rats in each group. Rabbits in all groups were modeled by bacterial endotoxin except group N. Rabbits in group W, A and TA were treated with acupuncture in Quchi(LI11), with no manipulation for group W, with lifting and thrusting manipulation during retention for group A, and with the same manipulation after elastase pretreatment for group TA, respectively. Content of IL-1β, TNF-α and IL-4 in serum were detected by ELISA, 2 hours, 4 hours and 6 hours after molding. 6 hours after molding, the relevant tissues were taken away to observe elastic fiber in LI11 with microscope. Results: Compared with group N, the contents of IL-1β, TNF-α increased significantly and IL-4 decreased significantly in group M(P<0.01); compared with group M, the contents of IL-1β, TNF-α decreased significantly and IL-4 increased significantly in group W and A(P<0.01, P<0.05); the contents of IL-1β, TNF-α of group A were lower than group W, while IL-4 was significantly higher(P<0.01); compared with group A, the contents of IL-1β, TNF-α increased significantly and IL-4 decreased significantly in group TA(P<0.01). Compared with group N, elastic fiber in point IL11 in group A appeared curlier. Conclusion: Acupuncture treatment can control inflammatory reaction caused by bacterial endotoxin, and acupuncture manipulation can greatly improve such effect. Elastic fiber at point area may be the important material basis of conducting mechanical stimulus during acupuncture.
