超声引导下穿刺抽吸与传统切开引流方法治疗乳腺脓肿的meta分析

【摘要】 目的:运用meta分析的方式评价超声引导下穿刺抽吸与直接切开引流方式治疗乳腺脓肿的临床效果。方法:按既定检索策略——计算机检索中外文数据库,包括中国期刊全文数据库(CNKI),万方数据库、Pub Med、Medline、Ebsco等;同时手工检索相关杂志。检索时间为建库至2017年9月,检索语言为中文、英文。收集所有符合条件超声引导下穿刺与切开引流治疗乳腺脓肿的随机对照试验(RCT),提取相关数据,进行统计学分析。结果:共纳入符合要求RCT文章11篇,共599例患者。meta分析显示:超声引导下穿刺抽吸治疗乳腺脓肿在治愈率[OR=1.85,95%CI(1.13,3.01),P=0.01],术后并发症发生率[OR=0.18,95%CI(0.07,0.45),P=0.00],切口愈合时间[SMD=-1.65,95%CI(-1.91,-1.39),P=0.00]、换药次数[SMD=-4.34,95%CI(-7.92,-0.76),P=0.02]等方面较传统切开引流方式的差异有统计学意义。结论:与传统切开引流术式相比,超声引导下穿刺引流技术在治愈率、术后并发症发生率、切口愈合时间、换药次数的比较上均占优势。

【Abstract】 Objective: To analyze and evaluate the clinical effect of ultrasound-guided needle aspiration(UGNA) and traditional incision and drainage(I&D) in the treatment of breast abscesses by meta analysis. Method: According to the retrieval strategy,the data base such as CNKI,WANFANG DATA,Pub Med,Medline,Ebsco etc. were searched by computer; meanwhile,the paper magazines were searched by manual within a date range from their inception up to September 2017. The language was limited to Chinese and English. Any randomized controlled trials(RCT)about comparison between UGNA versus I&D to treat the breast abscesses were retrieved and collected. Statistical analyses were carried out using Rev Man 5. 3. Results: 11 studies were included,involving 599 participants according to the inclusion criteria. UGNA was associated with more cure rate [OR = 1. 85,95 % CI(1. 13,3. 01),P = 0. 01] and lower rate of complications [OR = 0. 18,95 % CI(0. 07,0. 45),P = 0.0002〗. What’s more,UGNA was associated with less healing time [SMD =-1. 65,95 % CI(-1. 91,-1. 39),P < 0. 0001] and less times for changing the dressing [SMD =-4. 34,95 % CI(-7. 92,-0. 76),P = 0. 02]. Conclusions: Compared with I&D,UGNA has more cure rate,less healing time and less times for changing the dressing,and lower rate of complications.
