福多司坦治疗老年慢性阻塞性肺疾病急性加重期患者的临床疗效及其对呼出气一氧化氮、氧化应激及肺功能的影响

【摘要】 目的评估福多司坦治疗老年慢性阻塞性肺疾病(COPD)急性加重期患者的临床疗效及其对呼出气一氧化氮(FeNO)、氧化应激及对肺功能的影响。方法选取2017年2月至2017年8月本院收治的89例老年COPD急性加重期患者作为研究对象,根据随机数字表法对其进行分组,对照组44例患者采用常规治疗,福多司坦组45例患者在对照组基础上给予福多司坦治疗。记录两组患者治疗前后FeNO水平、氧化应激状态[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、血清丙二醛(MDA)]及肺功能[第1秒用力呼气容积占预计值的百分比(FEV1%)、肺活量(FVC)、第1秒用力呼气容积占肺活量的百分比(FEV1/FVC%)]指标并进行比较。结果治疗前福多司坦组患者FeNO水平与对照组相比较差异不明显,不具有统计学意义(P>0.05);治疗后两组患者FeNO水平均明显降低,且福多司坦组降低更为显著,具有统计学意义(P<0.05)。治疗后两组患者SOD、GSH-Px水平均明显升高,且福多司坦组升高更为显著,均具有统计学意义(P<0.05);治疗后两组患者MDA水平明显降低,且福多司坦组降低更为显著,具有统计学意义(P<0.05)。治疗后两组患者FEV1%、FVC、FEV1/FVC%均明显升高,且福多司坦组升高更为显著,均具有统计学意义(P<0.05)。在治疗期间两组患者不良反应均可耐受,福多司坦组治疗总有效率为95.56%(43/45)明显高于对照组81.82%(36/44),具有统计学意义(Z=3.283,P<0.05)。结论福多司坦治疗老年COPD急性加重期患者疗效可靠,对患者FeNO、氧化应激状态及肺功能具有明显的改善作用,值得在临床进行推广应用。

【Abstract】 Objective To discuss the clinical efficacy of fudosteine treated elderly patients with acute exacerbation of chronic obstructive pulmonary disease(COPD) and the effect of fudosteine on fractional exhaled nitric oxide(FeNO), oxidative stress, pulmonary function. Methods 89 elderly patients with acute exacerbation of COPD in our hosipital form Feb. 2017 to Aug. 2017 were selected as the research object,divided according to the randomly number table method,and 44 cases of patients in the control group were treated with routine treatment, while 45 patients in fudosteine group were given fuduositan treatment on the basis of the control group. The levels of FeNO,oxidative stress [Superoxide Dismutase(SOD),Glutathione peroxidase( GSH-Px), maleic dialdehyde( MDA) ] and lung function [ Forced expiratory volume in one second(FEV1%), forced vital capacity(FVC) were recorded and compared. The percentage of forced expiratory volume in 1 second to the forced vital capacity(FEV1/FVC%) ] before and after treatment in the two groups were analysed. Results Before treatment,there was no significant difference in the level of FeNO between the patients in the fudosteine group and the control group(P >0. 05). After treatment, the FeNO levels in the two groups significantly decreased, and the reduction in the fudosteine group was more significant than that in the control group(P < 0. 05). After treatment, the SOD, GSH-Px levels in the two groups significantly increased,and that in the fudosteine group increased more significantly than that in the control group(P <0.05). After treatment,the MDA levels in the two groups significantly decreased,and that in the fudosteine group decreased more significantly than tha the control group(P < 0. 05). After treatment, the FEV1%,FVC,FEV1/FVC% in the two groups significantly decreased, and those in the fudosteine group decreased more significantly than those in the control group(P <0.05). During the treatment,the two groups were tolerable in adverse reactions. The total effective rate of treatment in fudosteine group was 95.56%(43/45) and significantly higher than that in the control group 81. 82%(36/44), which was statistically significant(Z = 3. 283,P <0. 05). Conclusion The curative effect of fudosteine in treatment of elderly patients with acute exacerbation of COPD is reliable,has an obvious effect on the improvement of FeNO,oxidative stress and pulmonary function,it is worthy in clinical application.
