【摘要】 目的探讨乳腺癌钼靶X线特征与雌激素受体(ER)、孕激素受体(PR)表达的关系及诊断价值。方法选取因乳腺肿块接受手术治疗患者104例,均采用钼靶X线进行检查,与术后病理检查结果进行比较,同时采用免疫组织化学法测定乳腺癌组织ER和PR的表达情况。结果 104例患者经病理检查证实为乳腺癌患者63例,乳腺良性肿块患者41例;以钼靶X线BI-RADS分级中4b级为判断良恶性标准,其诊断灵敏度、特异度、阳性预测值、阴性预测值及准确度分别为77.78%、78.05%、84.48%、69.57%和77.78%;肿块边缘有毛刺,病变区密度为等或混杂密度及无结构扭曲患者ER、PR阳性表达率高于肿块边缘无毛刺、病变区密度为高密度、有结构扭曲患者,差异均有统计学意义(P﹤0.05)。结论钼靶X线在乳腺癌诊断中有一定的价值,其部分影像特征与ER和PR表达有关,可为指导内分泌治疗等提供依据。更多还原

【Abstract】 Objective To investigate the correlation between molybdenum target X-ray features and the expression of estrogen receptor(ER) and progesterone receptor(PR) in breast cancer and its diagnostic value.Method 104 patients with breast tumor which were treated by surgery were included in the study,molybdenum target X-ray was used for examination,of which the results were compared with postoperative pathological findings,besides,the expression of ER and PR in breast cancer tissues was detected by immunohistochemistry.Result In 104 patients,pathological confirmation of breast cancer was determined in of 63 cases,and breast benign tumor were observed in 41 cases; in molybdenum target X-ray BI-RADS classification,4 b was applied as the criterion for evaluating benign and malignant tumors,the diagnostic sensitivity,specificity,positive predictive value,negative predictive value and accuracy were 77.78%,78.05%,84.48%,69.57% and 77.78%,respectively; the positive expression of ER and PR in the patients with rough tumor margin,isodense or mixed density lesions and non-structural distortion were higher than that in patients with clear margin,highdensity lesion and structural distortions,the difference was statistically significant(P<0.05).Conclusion Molybdenum target X-ray is somewhat useful in the diagnosis of breast cancer,and some of the imaging features are related to the expression of ER and PR,which can be a guidance for endocrine therapy.更多还原
