【摘要】 目的探讨超声造影(CEUS)参数及动态血管模式曲线(DVPC)诊断前列腺结节良恶性的应用价值。方法选取于我院超声影像科接受CEUS检查的78例(102个结节)前列腺结节患者,根据穿刺活检结果,分为恶性组42例(54个结节)和良性组36例(48个结节)。比较2组DVPC以及时间强度曲线(TIC)相关参数。结果良性组上升时间(RT)和达峰时间(TTP)均大于恶性组,而峰值强度(IMAX)、上升支斜率(RS)及下降支斜率(DS)均小于恶性组(P均<0.05)。恶性组DVPC类型以正负双向波48.15%(26/54)和正向波27.78%(15/54)为主,良性组以负向波50.00%(24/48)为主(P<0.001)。结论分析TIC参数可了解前列腺良恶性结节的造影剂灌注特点,联合应用DVPC有利于鉴别前列腺良恶性结节。

【Abstract】 Objective To investigate the value of CEUS and DVPC in diagnosis of benign and malignant prostatic nodules.Methods Seventy-eight patients(102 nodules)with benign prostatic tuberculosis who underwent CEUS were enrolled.There were 42 cases(54 nodules)in malignant group and 36 cases(48 nodules)in benign group according to biopsy results.The parameters of DVPC and time intensity curve(TIC)were compared between two groups.Results Rise time(RT)and peak time(TTP)of benign group were higher than those of malignant group,while peak intensity(IMAX),rising branch slope(RS)and descending branch slope(DS)were lower than those of malignant group(all P <0.05).The biphasic wave of DVPC was 48.15%(26/54)and the positive phasic wave of DVPC was 27.78%(15/48)in malignant group,while the negative phasic wave of DVPC was 50.00%(24/48)in benign group(P<0.001).Conclusion TIC parameters can be analyzed to understand the contrast agent perfusion characteristics of benign and malignant prostatic,which combined with DVPC is conducive to the identification of benign and malignant prostatic nodules.
