针刺不同穴位对寒凝证类痛经大鼠子宫收缩程度及疼痛相关机制的影响

【摘要】 目的:比较针刺三阴交穴、关元穴对寒凝证类痛经大鼠子宫平滑肌收缩程度及脊髓、子宫组织中κ-受体、μ-受体含量的影响,探讨不同穴位介入的效应差异及部分机制。方法:将64只雌性SD大鼠随机分为盐水组、寒凝证类痛经模型组、针刺三阴交组、针刺关元组,每组16只。采用皮下注射苯甲酸雌二醇和低温冷冻的方法制备寒凝证类痛经模型,末次给药1 h后,麻醉大鼠,运用BL-420F生物机能实验系统记录子宫的收缩程度。采用Real Time-PCR检测大鼠脊髓及子宫组织中κ-受体、μ-受体的含量。结果:1.子宫平滑肌收缩程度:与盐水组比较,模型组子宫收缩波的个数显著增加(P<0.01)、子宫收缩波峰峰值显著升高(P<0.05)、子宫活动度显著增强(P<0.01),三阴交组、关元组子宫活动度显著增强(P<0.01,P<0.05);与模型组比较,三阴交组与关元组子宫收缩波的个数及子宫活动度均显著下降(P<0.05)。2.脊髓及子宫组织中κ-受体、μ-受体水平:(1)脊髓中的κ-受体及μ-受体水平:各组间比较无统计学差异;(2)子宫组织中的κ-受体水平:各组间比较无统计学差异;(3)子宫组织中μ-受体水平:与盐水组比较,关元组子宫组织中μ-受体水平显著升高(P<0.05);与模型组比较,三阴交组、关元组子宫组织中μ-受体水平均显著升高(P<0.05,P<0.01)。结论:针刺三阴交穴、关元穴均可降低子宫收缩波个数和子宫活动度,抑制子宫平滑肌的异常收缩反应。分析其效应的部分机制是通过调节子宫靶器官组织的阿片肽受体水平,进而达到针刺镇痛的作用。

【Abstract】 Objective: To compare the different effect of needling SP6 and CV4 on dysmenorrhea rats of cold congealing type,by observing the uterus contraction level,and the changes of κ opioid receptor and μ opioid receptor in the spinal cord and uterus tissues. Methods: 64 female SD rats were randomly divided into the saline control group,the model group,SP6 group( needling acupoint SP6),and CV4 group( needling acupoint CV4),with 16 rats in each group. The models were established by subcutaneously injecting Estradiol Benzoate and being exposed to freezing environment. One hour later after the injection of Estradiol Benzoate on the 10 th day,0. 7 ml/100 g Urethane was given by intramuscular injection for anesthesia. BL-420 F was used to record the uterus contraction. Real Time-PCR was applied to test the amount of κ opioid receptor and μ opioid receptor in the spinal cord and uterus tissues. Results: In terms of uterine contraction: compared to saline control group,the amount of the uterine contraction waves,peak wave values,and uterine activities were increased significantly in the model group,group SP6,and group CV4( P < 0. 05 or P < 0. 01); in which the needling groups improved significantly compared to the model group( P < 0. 05 or P < 0. 01). In terms of the changes of κ opioid receptor and μ opioid receptor,there was no significant difference on κ and μ opioid receptors in the spinal cord among the four groups; There was no significant difference on the κ opioid receptor in the uterus among the groups;compared to the saline control group,μ opioid receptor in the uterus increased significantly in group CV4( P < 0. 05);compared to the model group,μ opioid receptor in the uterus increased significantly in both group SP6 and group CV4( P < 0. 05 or P < 0. 01). Conclusion: Needling on SP6 and CV4 can both reduce the amount of uterus contraction waves and contraction degree to alleviate abnormal uterus contraction. The mechanism may be achieved by regulating the endogenous opioid receptor in uterus tissues.
