复方硫酸亚铁叶酸片对慢性心力衰竭合并缺铁性贫血患者的疗效

【摘要】 目的:探讨复方硫酸亚铁叶酸片对慢性心力衰竭(CHF)合并缺铁性贫血(IDA)患者的疗效。方法:选取100例CHF合并IDA患者,根据随机数字表法分为对照组和治疗组,每组各50例,所有患者予常规抗心力衰竭治疗,治疗组予复方硫酸亚铁叶酸片治疗6周。比较治疗前及治疗6周后两组患者心功能分级、N端B型脑钠肽前体(NT-pro BNP)、6 min步行试验、左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、血浆同型半胱氨酸(Hcy)等变化。结果:经过6周治疗后,两组患者心功能分级改善(P<0.05)、NT-pro BNP水平下降(P<0.05)、6 min步行距离增加(P<0.05)、LVEF升高(P<0.05),LVEDD缩小(P<0.05),治疗组更明显(P<0.01);治疗组Hcy下降(P<0.01)。结论:复方硫酸亚铁叶酸片可改善CHF合并IDA患者的心功能,机制之一可能是通过降低Hcy。

【Abstract】 Objective:To observe the effect of compound of sulfate ferrous and folic acid in chronic heart failure(CHF) patients with iron deficient anemia(IDA).Method:100 patients with CHF and IDA were randomly assigned to control group and treatment group,50 cases in each group.All of them were given regular anti-heart failure treatment,while the treatment group was given compound of sulfate ferrous and folic acid additionally.The cardiac function,N-terminal B-type natriuretic peptide(NT-pro BNP),six minute walking test,left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD) and concentration of homocysteine(Hcy) of two groups were detected before and after treatment.Result:After 6 weeks of treatment,NYHA cardiac function improved(P<0.05),NT-pro BNP decreased(P<0.05),the distance of six minutes walking test was longer(P<0.05),LVEF improved while LVEDD reduced(P<0.05),those difference of the indexes in the treatment group were more obviously than those in the control group(P<0.01).Hcy reduced in the treatment group(P<0.01).Conclusion:Compound of sulfate ferrous and folic acid can improve cardiac function of patients with CHF and IDA probably by decreases Hcy level.
