【摘要】 目的探讨基于微信平台的经外周静脉穿刺中心静脉置管(PICC)患者医院-社区-家庭全程化管理方案的构建与应用效果。方法选取2016年1月—2017年1月留置PICC导管的肿瘤患者115例设为观察组,另选取2015年1月—12月留置PICC的肿瘤患者120例设为对照组。对照组采用常规PICC门诊维护,观察组在对照组基础上实施基于PICC微信公众号的医院-社区-家庭全程化管理方案。观察2组PICC置管相关并发症发生率及患者对PICC维护指导的依从性。结果观察组血栓和非计划性拔管发生率低于对照组,差异有统计学意义(P<0.05)。观察组PICC维护指导依从性评分为(8.14±4.26)分,低于对照组的(18.14±6.57)分,差异有统计学意义(P<0.05)。结论医院-社区-家庭一体化PICC维护网络的建立,解决了出院肿瘤患者带管的维护问题,减少了带管期间并发症,提高了患者对PICC维护指导的依从性。更多还原

【Abstract】 Objective To evaluate the hospital-community-family whole-process management based on Wechat platform in patients with PICC. Methods A total of 120 cancer patients with peripherally inserted central catheter( PICC) were selected as the control group between January,2015 and December,2015,with routine nursing and maintenance. Another 120 cancer patients with PICC were enrolled as the observation group between January,2016 and January,2017,with hospital-community-family whole-process management based on Wechat platform. The incidence of PICC-related complications and patient’s compliance to PICC maintenance were compared between two groups. Results The incidence of PICC-related thrombus and unplanned removing of catheter was low er in the observation group than that of controls,with a significant difference( P < 0. 05). the score in assessment of PICC maintenance compliance was( 8. 14 ± 4. 26) in the observation group and was( 18. 14 ± 6. 57) in the control group,with a significant difference( P < 0. 05). Conclusion Hospital-community-family whole-process management based on Wechat platform provides a way of PICC maintenance,which is potentially effective to reduce the risk of incidence of PICC-related complications.更多还原
