人型支原体与阴道加德纳菌对细菌性阴道病感染的相关性分析

【摘要】 目的探讨人型支原体(Mycoplasma hominis,MH)在细菌性阴道病(bacterial vaginosis,BV)感染过程中的作用以及其与阴道加德纳菌(Gardnerella vaginalis,GV)之间潜在的相互作用。方法采用纽金特评分对260份女性分泌物标本进行BV检测,确认BV 41份,BV中介27份,非BV 192份。通过镜检,实时荧光定量核酸扩增检测试剂盒及培养法同时对女性分泌物标本中阴道加德纳菌、人型支原体、念珠菌(Canidia albicans,CA)及解脲脲原体(Ureaplasma urealyticum,UU)进行检测。结果 GV和MH的检出率与BV、BV中介和非BV呈现明显的相关性(r值分别为0.994、0.914,P<0.05);而UU和CA的检出率差异无统计学意义(P>0.05)。MH和GV在BV、BV中介和非BV组中呈现出递减趋势;MH和GV主要分布于BV和BV中介组;在非BV组中的MH的分布远小于GV,仅为9.4%。GV和MH在BV(78.1%)、BV中介(66.7%)混合检出率明显高于非BV组(7.8%)。GV和CA混合检出率在BV、BV中介与非BV组中依次降低。结论 MH在BV感染过程中起重要作用而且与GV存在潜在的协同作用。

【Abstract】 Objective To explore the role of Mycoplasma hominis(MH) in bacterial vaginosis(BV) and its potential patterns of synergy with Gardnerella vaginalis in women with BV. Methods Vaginal swabs from 260 women categorised by Nugent score confirmed BV in 41 cases,intermediate in 27 cases and non-BV in 192 cases. Gardnerella vaginalis,Mycoplasma hominis and Canidia albicans in vaginal swabs were tested by microscopic examination,q PCR and bacterial culture. Results The detection rates of Gardnerella vaginalis and Mycoplasma hominis displayed a significant positive correlation(r = 0. 994,r = 0. 914,P <0. 05) with BV,intermediate and non-BV; while the difference of detection rate between Ureaplasma urealyticum and Canidia albicans was not statistically significant(P > 0. 05). A decreasing trend of Gardnerella vaginalis and Mycoplasma hominis was showed in BV,intermediate and non-BV. Prevalence of Gardnerella vaginalis and Mycoplasma hominis were mainly in women with BV and intermediate. In addition,a less detection rate(9. 4%) of Mycoplasma hominis was displayed in non-BV compeared with Gardnerella vaginalis. The detection rate of Mycoplasma hominis and Gardnerella vaginal co-infection were higher in BV(78. 1%) and BV intermediate(66. 7%) than that in BV(7. 8%). While that of Mycoplasma hominis and Gardnerella vaginalis co-infection in women with BV,intermediate and non-BV decreased in order. Conclusion This experiment confirms the important role of Mycoplasma hominis in BV infection and its potential synergistic effect on Gardnerella vaginalis was observed.
