【摘要】 目的探讨维持性血液透析炎症因子水平与患者心功能及营养状态的关系。方法采用酶联免疫吸附法检测患者血液中IL-6、TNF-α水平,探讨血液透析患者炎症状态与患者心功能及营养状态的关系。结果 TNF-α高水平组患者左心房内径、左心室舒张末期内径、射血分数、B型脑钠肽等各指标水平均明显差于TNF-α低水平组患者;IL-6高水平组患者以上指标均明显差于IL-6低水平组患者,上述差异均有统计学意义(P<0.05)。TNF-α高水平组患者轻度营养不良占20%、中度营养不良占60%,重度营养不良占20%,明显差于TNF-α低水平患者;IL-6高水平组各营养指标均差于IL-6低水平组患者,TNF-a、IL-6水平与LAD、LVEDd、MIS、BNP评分成正相关,P<0.05;与EF成负相关,P<0.05。结论血液透析患者炎症因子水平与患者心功能及营养状态密切相关。更多还原

【Abstract】 Objective To investigate the relationship of inflammatory factors level with cardiac function and nutritional status in maintenance hemodialysis patients.Methods IL-6 levels and TNF-α levels in patients with hemodialysis were examined by enzyme-linked immunosorbent assay,and the relationship of inflammatory state with cardiac function and nutritional status in patients was discussed.Results The LAD,LVEDd,EF,BNP level in high level TNF-α group were lower to the low level TNF-α group; so was it in the high IL-6 group than the low level IL-6 group,with the difference statistically significant(P < 0.05).Patients with mild malnutrition accounted for 20%,moderate malnutrition accounted for 60%,and severe nutritional accounted for 20% in high TNF-α group,which were significantly worse than that in patients with low levels of TNF-α; nutritional index in the high level IL-6 group were lower than that in the low level group,with the difference statistically significant(P < 0.05).TNF-α and IL-6 levels were positively correlated with LAD,LVEDd,MIS scores(P < 0.05); while they were negatively correlated with EF,BNP(P < 0.05).Conclusion The level of inflammatory factors in hemodialysis patients is closely related to cardiac function and nutritional status.更多还原
