双层膜技术在上颌前牙区延期种植骨缺损修复中的应用

【摘要】 目的评价可吸收膜与不可吸收膜联合应用修复上颌前牙区延期种植骨缺损的临床效果。方法将42颗Ankylos种植体植入37例患者牙槽嵴较狭窄的上颌前牙区,种植体植入后唇侧均有不同程度螺纹暴露,形成的骨缺损空间维持能力较弱。植入羟基磷灰石珊瑚骨粉,表面覆盖钛膜,钛钉固定,将胶原膜覆盖在钛膜表面,松解骨膜,严密缝合创口。结果术后有2例患者出现创口裂开钛膜外露,钛膜暴露率为5.4%。所有种植体唇侧均无种植体螺纹暴露。结论钛膜和胶原膜的联合应用可显著减少钛膜的暴露率,推迟钛膜暴露的时间,使得成骨效果更具可预测性,使覆盖其上的牙龈组织外形轮廓理想,从而获得满意的美学效果和理想的生理功能。

【Abstract】 Objective To evaluate the clinical effect of guided bone regeneration technique combined with absorbable membraneand non-absorbable membrane for repairing bone defects around implants in the maxillary anterior.Methods42 Ankylos implants were implanted in 37 patient with a narrow alveolar in the maxillary anterior.After implantation,varying degrees of the threads were exposed in the labial side of implants and the bone defects were with less space making feature.The bone defects were filled with coral hydroxyapatite bone meal,and covered with titanium membrane fixed by titanium screw.Then collagen membrane was placed above the titanium membrane,release periosteum and sutured the wounds.Results after the surgery,the titanium membranes of 2cases were exposed,The exposed rate of tiatanium membrane was 5.4%.in the labial side of all implants there was no thread was exposed.Conclusion The combined application of titanium membrane and collagen membrane significantly reduce the exposure rate of titanium membrane,delay the titanium membrane exposure time,makes osteogenesis more predictable,and makes contour of the above gingival tissue ideal,obtaining a satisfactory physiological effect and desired aesthetic function.
