0~6岁儿童神经心理发育量表在儿童智力障碍诊断中的临床应用

【摘要】 目的探讨0~6岁儿童神经心理发育量表在儿童智力障碍诊断中的临床应用效果。方法 9539例参加健康体检的6个月~5岁儿童应用0~6岁儿童神经心理发育量表进行智力评估并记录其胎龄及出生体重。结果测评后,被测试儿童中智力低下儿童所占比例为2.76%(263/9539);早产儿组总体智力水平得分为(89.18±11.54)分,足月儿组总体智力水平得分为(96.46±7.96)分,足月儿组发育商明显高于早产儿组(P<0.01)。被测儿童总体智力水平得分为(94.96±9.59)分。足月儿中出生体重<2.5 kg组儿童智力评分为(93.22±9.95)分,≥2.5 kg组儿童智力评分为(96.60±7.81)分,足月儿中出生体重<2.5 kg组儿童智力评分发育商明显低于出生体重≥2.5 kg组(P<0.01)。263例智力低下儿童各领域发育商(DQ)<70分中,大动作领域落后145例(55.13%),精细动作领域落后217例(82.51%),适应能力领域落后220例(83.65%),语言领域落后232例(88.21%),个人-社交行为领域落后217例(82.51%)。本组智力障碍患儿中被怀疑为孤独症谱系障碍者所占比例最高,达20.15%。结论早产、出生体重低等因素对智力发育有较大影响。智力障碍患儿中以语言发育落后为主要表现者多见。智力障碍患儿常常伴随特殊性的表现。

【Abstract】 Objective To investigate clinical application effect by 0~6-year-old children neuropsychological development scale in diagnosis of children dysgnosia. Methods A total of 9539 children ageing 6-month old~5-year-old in physical examination received 0~6-year-old children neuropsychological development scale for intelligence evaluation, and their gestational age and birth weight were recorded. Results After evaluation, mental retardation children accounted for 2.76%(263/9539) in all children in research. Premature infants group had overall intelligence score as(89.18±11.54) points, and term infants group had overall intelligence score as(96.46±7.96) points. Term infants group had obviously higher development quotient than premature infants group(P<0.01). Overall intelligence score of all children was(94.96±9.59) points. In term infants, birth weight < 2.5 kg group had intelligence score as(93.22±9.95) points, and birth weight ≥ 2.5 kg group had intelligence score as(96.60±7.81) points. Birth weight < 2.5 kg group in term infants had obviously lower development quotient than birth weight ≥ 2.5 kg group(P<0.01). Among 263 children with dysgnosia and each field DQ < 70 points, there were 145 cases(55.13%) in laggard major movement field, 217 cases(82.51%) in laggard fine movement field, 220 cases(83.65%) in laggard adaptive capacity field, 232 cases(88.21%) in laggard language field, and 217 cases(82.51%) in laggard personal-social behavior field. Children with dysgnosia in this research showed higher proportion as 20.15% with suspected autism spectrum disorder. Conclusion Premature and low birth weight show huge influence on intelligence development. Children with dysgnosia have major manifestation as delayed language development. Special manifestations are common in children with dysgnosia.
