【摘要】 目的了解欠发达地区开展腹股沟疝日间手术的困难所在,从而指导制定相关的改进措施。方法调查南充市中心医院·川北医学院第二临床学院全体41名医师护士和2014年11月至2015年9月于我科住院治疗的253例成年腹股沟疝患者和279例家属对腹股沟疝日间手术的了解和支持率及其原因。结果医师对腹股沟疝日间手术的了解率和支持率分别为100%和57.14%,护士88.89%和88.89%,患者9.88%和25.30%,家属12.90%和36.20%。不同文化程度的患者和家属,对腹股沟疝日间手术的了解率、支持率比较,差异有统计学意义(χ~2=25.519、10.166,P<0.001、=0.006)。医护对腹股沟疝日间手术的顾虑主要为可能引发不必要的医患纠纷,而患者和家属担心之处主要为医保政策不明,术后并发症的处理,过早活动增加复发率。结论欠发达地区医师和护士对腹股沟疝日间手术的了解率和支持率较高,但患者和家属的了解率和支持率较低,且与文化程度密切相关。欠发达地区腹股沟疝日间手术的发展尚需社会各界的共同努力。更多还原

【Abstract】 Objective To understand the difficulty of inguinal hernia day surgery in underdeveloped regions,so as to make a guidance for improvement measures. Methods 253 inguinal hernia patients who were hospitalized in Second Department of General Surgery,Nanchong Center Hospital,during November 2014 to September 2015,as well as their 279 families,all 41 surgeons and nurses were enrolled. They had been asked to answer a questionnaire about if they knew or approved inguinal hernia day surgery and the reasons. Results The knowing and approval rate of surgeons to inguinal hernia day surgery were 100% and 57. 14%; the nurses were 88. 89% and 88. 89%; the patients were 9. 88% and 25. 30%; the family members were 12. 90% and36. 20%,respectively. The patients and their families with different educational level had different knowing rate and support rate of inguinal hernia day surgery,and the difference were significant( χ2= 25. 519,P <0. 001; χ2= 10. 166,P = 0. 006). The main concern of surgeons and nurses to inguinal hernia day surgery was that it may lead to unnecessary doctor-patient dispute,while the patients and their families worried about the unclear medical insurance policy,the treatment of postoperative complications and whether the early activity would increase the recurrence rate. Conclusion The surgeons and nurses in underdeveloped regions have higher rates of knowing and support to inguinal hernia day surgery,but patients and their families have lower rates of knowing and support,which are closely related to education level. The development of inguinal hernia day surgery in underdeveloped regions still needed the joint effort of all sectors of society.更多还原
