1型糖尿病与2型糖尿病对精子质量的影响

【摘要】 目的探讨1型糖尿病(T1DM)和2型糖尿病(T2DM)对男性精子的影响。方法选取1型糖尿病患者36例,2型糖尿病患者73例,健康对照者116名,分别予以检测分析其激素水平、精液常规、精子碎片指数和精子核蛋白组型,以评价糖尿病对男性精子参数的影响情况。结果促间质细胞激素(LH)、睾酮(T)和平均精子浓度:三组比较差异无统计学意义;卵泡刺激素(FSH)、平均精子DNA碎片指数(DFI)和精子核蛋白组型转换异常比例:两个DM组患者的检测结果均较健康对照组升高(P<0.01);精液量和正常形态精子百分率,两个DM组患者均较健康对照组降低(P<0.01,P<0.05);前运动精子比例:两个DM组均较健康对照组降低,其中T1DM组与对照组比较差异有统计学意义(P<0.05)。结论1型和2型糖尿病对FSH、平均精子浓度、精子核蛋白组型转换异常比例、精液量、正常形态精子百分率、前运动精子比例等参数均有影响,从而导致男性生育力的下降,其中T1DM组影响更明显。

【Abstract】 Objective To investigate the effects of type 1 diabetes mellitus(T1 DM)and type 2 diabetes mellitus(T2 DM)on male sperm.Methods Thirty-six patients with type 1 diabetes mellitus and 73 patients with type 2 diabetes mellitus,and 116 healthy controls were enrolled in this study.Hormone levels,sperm motility,sperm fragmentation index and sperm nuclear protein type of the subjects were determined to evaluate the effects of diabetes on sperm parameters.Results There was no significant difference among the three groups on LH,T and average sperm concentration;but FSH,mean sperm DFI and sperm nuclear protein type conversion ratio were abnormal,and were significantly higher in the two DM patients than the healthy control group(P<0.01).The sperm quantity and the percentage of normal spermatozoa in both DM groups were lower than those in healthy control group(P<0.01,P<0.05);the ratio of pre-exercise sperm in the two DM groups were lower than the healthy control group,the difference with the control group was significant(P<0.05).Conclusion Type 1 and type 2 diabetes affect the parameters such as FSH,average sperm concentration,aberrant sperm nuclear transfer rate,semen volume,normal spermatozoon percentage and pre-sperm ratio,which lead to the decline of male fertility,and the effect of T1 DM group is more obvious.
