目的前瞻性分析我院血流感染病原菌及多重耐药菌情况,为血流感染早期合理选用抗菌药物提供依据。方法对我院2016年1月—2017年6月住院患者血培养标本进行培养,对阳性报警标本进行分离、鉴定和药敏分析,医院感染管理办公室人员每天收集血流感染患者的临床资料。结果共收集非重复病原菌569份,其中革兰阳性菌(48.86%)稍多于革兰阴性菌(44.46%)。前5位细菌依次为:大肠埃希菌、凝固酶阴性葡萄球菌、金黄色葡萄球菌、链球菌属和肺炎克雷伯菌。病区分布以重症加强护理病房(intensive care unit,ICU)、儿科、新生儿科、血液科和心胸外科为主,年龄分布60岁以上老年人比例最高(37.08%)。导管相关血流感染病原体以葡萄球菌属为主。多重耐药菌(MDRO)占46.40%,其中社区感染比例(56.82%)高于医院感染(42.42%)。MDRO比例超过50%的细菌是:凝固酶阴性葡萄球菌(68.57%)、大肠埃希菌(63.47%)和肠球菌(52.00%)。大肠埃希菌和肺炎克雷伯菌对亚胺培南耐药率分别为3.59%和7.40%,产ESBL株检出率分别为58.08%和7.41%,该两种细菌耐药率低于30%的药物包括阿米...
