社区和医院获得性耐甲氧西林金黄色葡萄球菌MLST联合SCCmec基因分型及耐药性比较
Drug resistance and gene typing of community-associated methicillin-resistant Staphylococcus aureus and hospital-associated methicillin-resistant Staphylococcus aureus
【摘要】 目的了解社区获得性耐甲氧西林金黄色葡萄球菌(CA-MRSA)和医院获得性耐甲氧西林金黄色葡萄球菌(HAMRSA)分离株的耐药性及遗传背景。方法对CA-MRSA和HA-MRSA进行多位点序列分型(MLST)检测,应用多重聚合酶链式反应(PCR)技术对CA-MRSA和HA-MRSA进行SCCmec基因分型;采用Walk WAY96PLUS PC33药敏板联合K-B纸片扩散法检测CA-MRSA和HA-MRSA抗菌药物敏感性。结果 63株HA-MRSA共检测出2个基因型,4株CA-MRSA共检测出4个基因型,二者遗传背景完全不同。HA-MRSA对红霉素、克林霉素、四环素、庆大霉素、左氧氟沙星和环丙沙星耐药率显著高于CAMRSA(均P<0.05)。结论滨州医学院附属医院CA-MRSA和HA-MRSA分型及主要流行株存在差异,CA-MSSA药物敏感性明显高于HA-MRSA;MLST联合SCCmec基因分型方法是MRSA分子流行病学分析的有效方法。
【Abstract】 Objective To investigate the drug resistance and genetic background of hospital-associated and community-associated methicillin-resistant Staphylococcus aureus(HA-MRSA and CA-MRSA). Methods The multilocus sequence typing(MLST) technique was introduced to identify sequence types(ST) of HA-MRSA and CA-MRSA, and the staphylococcal cassette chromosome mec(SCCmec) types were determined by multiplex PCR methods. The susceptibility to antimicrobial agents was tested by the Walk WAY96 PLUS PC33 and Kirby-Bauer methods. Results Among the 67 isolates of MRSA, two types of gene isolates belonged to HA-MRSA; four gene typing isolates belonged to CA-MRSA. The resistance rates to erythromycin, clindamicin, tetracycline, gentamicin,levofloxacin, and ciprofloxacin of HA-MRSAwere higher than those of CA-MSSA(P<0.05). Conclusion The differences between hospital-and community-acquired Staphylococcus aureus were significant in terms of gene typing in Binzhou Medical University Hospital. CA-MSSA was more sensitive to all antibiotics than HA-MRSA. MLST combined with SCCmec types is a rapid and simple MRSA genotyping method.
