前弹力层下准分子激光原位角膜磨镶术矫正成人近视性屈光参差对双眼视觉的影响
The analysis of binocular stereopsis after SBK in adults with myopic anisometropia
【摘要】 目的观察前弹力层下准分子激光原位角膜磨镶术(SBK)矫正成人近视性屈光参差后双眼近立体视的变化。方法选择近视性屈光参差患者48例(90眼),其中高度屈光参差组32例(60眼),重度屈光参差组16例(30眼),术后随访6个月以上。比较患者手术前后裸眼视力、最佳矫正视力及双眼近立体视的变化。结果术前屈光参差程度(4.75±2.83)D,术后为(0.35±0.15)D。SBK术后6个月84眼裸眼视力达到1.0或以上,较术前戴框架眼镜的78眼明显提高。术前戴框架眼镜的正常立体视、异常立体视和立体视盲分别为15、15和18例,而SBK术后6个月正常立体视、异常立体视和立体视盲分别为28、14和6例;SBK术后6个月的近立体视明显优于术前戴框架眼镜,差异有统计学意义(P<0.01)。高度屈光参差组近立体视明显优于重度屈光参差组,差异有统计学意义(P<0.01)。结论 SBK有助于成人近视性屈光参差患者双眼近立体视的建立。早期干预近视性屈光参差患者的戴镜习惯,有助于患者近立体视的建立。
【Abstract】 Objective To investigate the changes of near stereopsis after sub-Bowman-Keratomileusis(SBK) treatment in adults with myopic anisometropia. Methods Forty eight patients(90 eyes) including 32 cases(60 eyes) of high anisometropia group and 16 cases(30eyes) of severe anisometropia underwent SBK and were followed up for six months. The uncorrected visual acuity(UCVA), the best corrected visual acuity(BCVA), near stereopsis were measured and compared before and after SBK. Results The degree of pre-operative anisometropia was(4.75±2.83) D and that of post-operation was(0.35±0.15)D. The UCVA were reached 1.0 or better in 84 naked eyes in six months after SBK, while more than 78 eyes wore glasses pre-operatively. Among 48 patients wearing glasses previously, there were 15 cases of normal stereopsis, 15 cases of abnormal stereopsis and 18 cases of stereo blindness. In six months after SBK, there were 28 cases of normal stereopsis, 14 cases of abnormal stereopsis and 6 cases of stereo blindness. The near stereopsis after SBK was significantly higher than that of pre-operation(P<0.01). The near stereopsis in high anisometropia patients was significantly better than that in severe anisometropia patients(P<0.01). Conclusion SBK is useful for adult patients with myopic anisometropia to build near stereopsis. Early intervention of wearing habits in patients with myopic anisometropia can help to establish normal near stereopsis.
