【摘要】 目的探讨耳内镜下听骨链触动试验在慢性化脓性中耳炎术前评估中的应用。方法对38例(39耳)慢性化脓性中耳炎患者手术前予耳内镜检查、内镜下听骨链触动、纯音测听、鼓膜贴补试验、咽鼓管功能及颞骨CT等检查,根据检查结果选择不同的手术方式;将术前耳内镜下听骨链触动试验的结果、鼓膜贴补试验的结果及术中显微镜下探查听骨链的活动情况进行比较分析。结果耳内镜下听骨链触动试验对慢性化脓性中耳炎听骨链病变的诊断灵敏度、特异度、准确度分别是89.5%、80%、84.6%。鼓膜贴补试验的是50%、57.59%、54.5%。结论不同角度的耳内镜对中耳鼓室结构、听骨链活动情况进行观察,与手术中显微镜下所见有较高的符合率,术前使用耳内镜评估慢性化脓性中耳炎听骨链病变有重要的临床应用价值。更多还原

【Abstract】 Objective To report use of ossicles tapping test under ear endoscopy during preoperative assessment for chronic suppurative otitis media.Methods A total of 38 patients(39 ears) with chronic suppurative otitis media were enrolled.Preoperative examination included ear endoscopy,endoscopic ossicles tapping test,pure tone audiometry,tympanic membrane patching test,eustachian tube function test,temporal bone CT,etc.Operative methods were chosen based on examination results.The relationship between ossicles tapping test and tympanic membrane patching test before surgery and the actual ossicular mobility observed during surgery was analyzed.Results The sensitivity,specificity and accuracy of ossicles tapping test were 89.5%,80% and 84.6%,respectively,and only 50%,57.59% and 54.5%,respectively,for tympanic membrane patching test.Conclusions Assessment of the tympanic cavity and ossicular chain mobility using ear endoscopy with different angle shows a promising correlation with actual surgical findings.Preoperative ear endoscopy therefore has important clinical values in evaluation of ossicular chain conditions in chronic suppurative otitis media.更多还原
