【摘要】 目的探讨碘伏和葡萄糖酸氯己定两种标准消毒液对中心静脉置管相关性感染的不同影响。方法选取2010年1月5日至2013年5月2日间江苏省盐城市第二人民医院1064例经锁骨下中心静脉置管肿瘤患者,将2011年9月10日至2013年5月2日间使用葡萄糖酸氯己定消毒者为氯己定组,2010年1月5日至2011年9月9日间使用碘伏消毒者为碘伏组,每组532例。由专人穿刺置管、专人护理和每日观察记载感染情况,对比两组患者发生感染几率。结果 1064例患者中,共发生感染23例(2.2%),均出现于碘伏消毒组,感染发生率为4.3%(23/532),包括局部感染14例(2.6%)、血液感染2例(0.4%)、可疑血液感染6例(1.1%)和局部合并血液感染1例(0.2%)。氯己定消毒组均未发生感染,两组对比,差异有统计学意义(P<0.001)。结论经锁骨下中心静脉置管相关性感染以局部感染为主,穿刺局部皮肤消毒及术后护理为首要环节,消毒液以氯己定为首选。更多还原

【Abstract】 Objective To explore the effect of two different standard disinfectants,chlorhexidine gluconate and povidone-iodine on central venous catheter related infections.Methods Select 1064 patients with cancer undergoing subclavian central venous catheter at Yancheng Second People’s Hospital from January 5,2010 to May 2,2013.Patients using chlorhexidine gluconate from September 10,2011 to May 2,2013 were included as the experimental group and patients using povidone-iodine from January 5,2010 to September 9,2013 were included as the control group with 532 patients in each group.Catheterization,personal care,daily observation and infection document were performed by specially appointed person.Results Among 1064 patients,infection occurred in a total of 23(2.2%) patients from povidone-iodine group and the infection rate was 4.3%(23/532) including local infection in 14 patients(2.6%),blood infection in 2 patients(0.4%),suspicious blood infection in 6 patients(1.1%) and partial merger blood infection in 1patient(0.2%).However,no patient was infected in chlorhexidine gluconate group(P <0.001).Conclusion The main catheter-related infection is local infection.The first thing to do is skin degerming and postoperative nursing and chlorhexidine is the best choice in all the disinfectants.更多还原
