心肺运动试验对疑似冠心病患者的冠脉评估初探
A preliminary study on coronary artery in patients with suspected coronary heart disease under cardiopulmonary exercise test
【摘要】 目的探讨心肺运动试验中氧摄取动力学、无氧阈及心肺储备功能对疑似冠心病患者冠状动脉狭窄的评估价值。方法选取本院100例疑似冠心病患者行心肺运动试验和冠脉CT检查,根据冠脉CT提示的冠脉狭窄程度进行分组,冠脉狭窄程度大于50%为冠脉狭窄组,冠脉狭窄程度小于等于50%为冠脉正常组,观察两组心肺运动试验中峰值时最大摄氧量、公斤摄氧量、氧脉搏、最大心率、心率储备,无氧阈下的公斤摄氧量、二氧化碳通气当量、氧脉搏,以及运动后心率恢复情况。结果反映心输出量和心脏储备功能的峰值时最大公斤摄氧量、氧脉搏、无氧阈下的二氧化碳通气当量冠脉狭窄组明显高于冠脉正常组,反映有氧运动能力和心脏迷走神经活性的无氧阈值、心率恢复情况冠脉狭窄组明显高于冠脉正常组,差异均有显著性(P<0.05)。结论心肺运动试验中峰值时代谢指标最大公斤摄氧量、氧脉搏、做功效率、无氧阈值、无氧阈下的二氧化碳通气当量以及心率恢复可能与冠脉狭窄程度有关。
【Abstract】 Objective To evaluate the value of oxygen uptake kinetics, anaerobic threshold and cardiopulmonary reserve in cardiopulmonary exercise test for coronary artery stenosis in patients with suspected coronary artery disease. Method 100 patients with suspected coronary heart disease were selected for cardiopulmonary exercise test and coronary CT, grouped according to the degree of coronary stenosis suggested by coronary CT, Coronary stenosis greater than 50% of the coronary stenosis group, the degree of coronary stenosis less than 50% of the normal coronary group, The maximal oxygen uptake, kilogram of oxygen uptake, oxygen pulse, maximal heart rate and heart rate reserve were observed at peak values and kilogram of oxygen uptake, carbon dioxide ventilation equivalent, oxygen pulse were observed under Anaerobic threshold, and heart rate recovery after exercise both groups of cardiopulmonary exercise trials. Result There was significant difference in peak oxygen uptake, oxygen pulse, efficiency, anaerobic threshold, anaerobic threshold of carbon dioxide ventilation equivalent and heart rate recovery in the coronary stenosis group compared with the normal coronary group. Conclusion Patients with coronary stenosis greater than 50% may have a metabolic index at the peak of the cardiopulmonary exercise test with maximum oxygen uptake, oxygen pulse, efficiency, anaerobic threshold, anaerobic subthreshold of carbon dioxide ventilation equivalent and heart rate recovery.
