曲安奈德药物涂层可降解支架对胆管损伤后瘢痕愈合的影响
Effect of triamcinolone-eluting biodegradable stent on scar healing of bile duct injury
【摘要】 目的 观察曲安奈德药物涂层可降解支架对胆管损伤后瘢痕愈合的影响。方法 选取24只清洁级新西兰雄性大白兔完全随机分为观察组和对照组,每组12只。建立胆管损伤模型后,观察组于胆总管内置入曲安奈德药物涂层可降解支架、对照组于置入普通可降解胆道支架。支架置入后60 d,检测损伤胆管组织中胶原、成纤维细胞分子、Toll样受体4及炎症通路分子、细胞凋亡因子的mRNA表达量。结果 支架置入后60 d时,观察组胆总管组织中Ⅰ、Ⅱ、Ⅲ型胶原、转化生长因子β1、结缔组织生长因子、α-平滑肌肌动蛋白、Toll样受体4、核因子KB、肿瘤坏死因子α、白细胞介素6、细胞凋亡抑制蛋白1的mRNA表达量均明显低于对照组[(0.37±0.06)比(1.03±0.15)、(0.41±0.05)比(1.01±0.17)、(0.28±0.04)比(0.98±0.14)、(0.33±0.05)比(1.06±0.12)、(0.46±0.07)比(1.03±0.14)、(0.36±0.05)比(0.98±0.14)、(0.28±0.05)比(0.98±0.12)、(0.39±0.06)比(1.05±0.17)、(0.56±0.07)比(1.02±0.14)、(0.30±0.06)比(0.97±0.15)、(0.33±0.08)比(1.05±0.17)],半胱天冬酶3、7、9的mRNA表达量均明显高于对照组[(3.25±0.62)比(0.97±0.13)、(2.77±0.49)比(1.02±0.14)、(2.91±0.52)比(1.08±0.19)],差异均有统计学意义(均P<0.05)。结论 曲安奈德药物涂层可降解支架对胆管损伤后瘢痕愈合具有抑制作用。
【Abstract】 Objective To observe the effect of triamcinolone-eluting biodegradable stent on scar healing of bile duct injury. Methods Totally 24 New Zealand white rabbits were randomly divided into observation group(n= 12) and control group( n = 12). After establishment of bile duct injury models, the observation group was implanted triamcinolone-eluting biodegradable stent and the control group was implanted normal degradable stent in common bile duct. Expressions of collagen,fibroblast molecules,Toll-like receptor-4 channel factors and cell apoptosis factors were analyzed 60 d after stent implantation. Results Expressions of collagen Ⅰ,Ⅱ,Ⅲ,transforming growth factor-β1, connective tissue growth factor, α-smooth muscle actin, Toll-like receptor-4,nuclear factor-κB, tumor necrosis factor-α, interleukin-6 and cellular inhibitor of apoptosis protein-1 mRNA in common bile duct in observation group were significantly lower than those in control group [( 0. 37 ± 0. 06) vs(1.03 ±0. 15),(0.41 ±0.05) vs(1.01 ±0. 17),(0.28 ±0.04) vs(0.98 ±0. 14),(0.33 ±0.05) vs(1.06±0. 12),(0.46 ±0.07) vs(1.03 ±0. 14),(0. 36 ±0.05) vs(0.98 ± 0. 14),(0.28 ±0.05) vs(0.98 ±0. 12),(0. 39 ±0.06) vs(1.05 ±0. 17),(0.56 ±0.07) vs(1.02 ±0. 14),(0.30 ±0.06) vs(0. 97 ±0. 15),(0. 33 ± 0. 08) vs( 1. 05 ± 0. 17) ]( P < 0. 05); expressions of cysteinyl aspartate specific proteinase(caspase)-3, caspase-7 and caspase-9 mRNA in observation group were significantly higher than those in control group[(3.25±0.62) vs(0.97±0.13),(2.77±0.49) vs(1.02±0.14),(2.91±0.52) vs(1. 08 ± 0. 19)](P < 0. 05). Conclusion Triamcinolone-eluting biodegradable stent can inhibit scar healing of bile duct injury.
