S100A9蛋白在继发性肺结核进展加重监测中的应用价值

【摘要】 目的 探讨S100A9蛋白在继发性肺结核进展加重中的应用价值。方法 选取2015年3—12月北京市结核病胸部肿瘤研究所已确诊为继发性肺结核的患者143例,根据病情分为重症继发性肺结核组(72例)及轻症继发性肺结核组(71例),选取同期健康体检者作为正常对照组(41例)。对受试者外周血血浆标本应用酶联免疫吸附试验方法测定S100A9蛋白水平,免疫比浊法测定高敏C反应蛋白水平,魏氏法测定红细胞沉降率。采用Logistic回归方法分析上述指标与继发性肺结核严重程度的相关性,对3个指标在肺结核进展加重评估中的价值进行评价。结果 重症继发性肺结核组患者的S100A9蛋白、高敏C反应蛋白、红细胞沉降率水平均明显高于轻症继发性肺结核组及正常对照组(均P<0.05)。多因素Logistic回归分析结果显示S100A9蛋白及红细胞沉降率为继发性肺结核进展加重的独立危险因素(均P<0.05)。S100A9蛋白区分重症及轻症肺结核的敏感度为91.5%,特异度为86.7%。结论 继发性肺结核患者血浆S100A9蛋白在监测病情进展方面具有良好的临床应用价值。

【Abstract】 Objective To explore the application value of S100A9 protein in monitoring the progression of secondary pulmonary tuberculosis. Methods Totally 143 patients with secondary pulmonary tuberculosis from March to December 2015 in Beijing Tuberculosis and Thoracic Tumor Research Institute were divided into severe group(72 cases) and mild group(71 cases); 41 healthy people were enrolled as control group. The plasma concentration of S100A9 protein was tested by enzyme linked immunosorbent assay; the level of high sensitivity C-reactive protein(hs-CRP) was tested by immunoturbidimetry; erythrocyte sedimentation rate( ESR) was tested by Westergren method. The relevance among S100A9 protein, hs-CPR, ESR and the severity of secondary pulmonary tuberculosis was analyzed. Results Levels of S100A9 protein, hs-CRP and ESR in severe group were significantly higher than those in mild group and control group(P < 0. 05). Multivariate logistic regression analysis showed that S100A9 protein and ESR were independent risk factors of severe pulmonary tuberculosis(P < 0.05). The sensitivity and specificity of S100A9 protein in differential diagnosis of severe and mild pulmonary tuberculosis were 91. 5% and86. 7%, respectively. Conclusion Detection of plasma S100A9 protein level helps monitoring the progression of secondary pulmonary tuberculosis.
