宁波地区妇女宫颈感染HPV58分布与变异谱系分析

【摘要】 目的了解宁波地区人乳头瘤病毒高危亚型HPV58的型别分布和癌相关基因E6、E7区基因变异谱系特征。方法收集调查社区和医院就诊妇女的宫颈脱落细胞样本进行HPV分型检测,并对其中HPV58亚型阳性株进行癌相关基因测序和序列分析。结果778例社区人群样本HPV58阳性16例,检出率2.06%,1 835例就诊人群样本HPV58阳性74例,检出率4.03%;HPV58病毒株E6、E7基因序列分别成功获得6和11条,E6区有2个位点发生碱基颠换:C307T和A388C,E7区有5个位点发生碱基颠换:G694A、T726C、T744G、G761A和T803C;系统进化树分析显示宁波地区HPV58变异株主要位于该亚型基因变异谱系A的A1和A2分支。结论宁波人群HPV58感染率较高,多数感染者携带病毒的癌相关基因发生变异,应加强对人群HPV感染及其持续感染状况的监测,尤其是HPV58阳性人群。

【Abstract】 Objective To understand the vervical distribution of human papillomavirus type 58(HPV58)and the variant lineage of oncogene E6 and E7in Ningbo. Methods The cervical exfoliated cells were collected from women in community health centers and hospitals in Ningbo and HPV subtype was tested.The oncogenes of HPV58 positive strains were sequenced and analyzed. Results Out of total 778 samples from community health centers and 1 835 from hospitals,16 and 74were HPV58 positive with the positive rate of 2.06% and4.03%,respectively.6 E6 gene and 11 E7 gene were identified.C307 Tand A388Cbase transversion were found in HPV58 E6gene;G694A,T726 C,T744G,G761 Aand T803Cbase transversion were also found in HPV58 E7 gene.Analysis of the phylogenetic tree of HPV58 isolates in Ningbo showed that the prevalent HPV58 variants in Ningbo belonged to A1 and A2of lineage A. Conclusions The infection rate of HPV58 is relatively high in Ningbo,and the mutation of oncogene of HPV58 strains are common.The surveillance on HPV infection should be strengthened,especially for those HPV58 positive.
