重病监护病房鲍曼不动杆菌消毒剂耐药基因检测

【摘要】 目的了解江苏省内医院ICU鲍曼不动杆菌(Ab)药敏及消毒剂耐药基因携带情况。方法用PCR法检测2016年7家医院ICU采集的224株Ab的qac E△1-sul I基因携带情况,采用VITEK2系统对菌株进行药物敏感分析。结果徐州市F医院ICU采集的携带qac E△1-sul I基因的Ab最多(30株)。药敏分析结果显示除哌拉西林他唑巴坦等6种抗菌药物外,采集的Ab对其他抗菌药物的耐药率均高于50%。卡方检验结果显示qac E△1-sul I基因阳性菌株和阴性菌株,除氨苄西林等5种抗菌药物外,对其他的抗菌药物的耐药率差别均存在显著的统计学差异。结论徐州F医院ICU采集的鲍曼不动杆菌qac E△1-sul I基因携带率较高,qac E△1-sul I基因不仅与磺胺类抗菌药物耐药有关,与其他抗菌药物存在统计学关联。医院应加强针对性消毒措施。

【Abstract】 Objective To investigate the drug susceptibility of Acinetobacter baumannii( Ab) sampling from ICU of hospital in Jiangsu Province and the situation of Ab carrying the disinfectant-resistant gene. Methods The PCR method was used to detect the qac E△1-sul I gene in 224 strains of Ab collected from ICU of 7 hospitals in 2016. The drug susceptibility of the Ab was carried out by VITEK2 system. Results The number of Ab strains carried qac E△1-sul I in the ICU of Xuzhou F Hospital was the most. None carried qac E△1-sul I was found in the ICU of Suzhou E Hospital. Antimicrobial susceptibility showed that the resistance rates of the sampled Ab to other tested antibiotics were higher than 50% except for Piperacilline et al. 6 antibacterial drugs. Results of chi-square test showed that the qac E △ 1-sul I gene positive strains and the qac E △ 1-sul I gene negative strains had significant different resistance rates to other tested antibiotics except for Ampicillin et al. 5 antibacterial drugs. Conclusion The detection rate of qac E △ 1-sul I gene in the samples of ICU of Xuzhou F Hospital is the most. Qac E △ 1-sul I gene is not only related to the resistance of sulfonamides,but also statistically related to other antimicrobial agents. Disinfection measures should be strengthened in hospital.
