连续3年多重耐药菌医院感染监测与经济负担评价

【摘要】 目的了解多重耐药菌流行状况及多重耐药菌感染对住院日及住院费用的影响。方法回顾性调查2013-2015年多重耐药菌感染患者相关资料,分析主要感染部位与感染病原菌,比较感染患者与非感染患者住院日与住院费用差异。结果多重耐药菌感染主要发生在呼吸系统、血液系统以及器械相关感染,构成比分别为50.91%、12.21%及11.43%;3年多重耐药菌感染菌株检出率为30.2%,年度检出率逐年下降。多重耐药菌感染患者住院日较非感染者长20 d(P<0.001),住院总费用增加89 822.66元(P<0.001)。结论多重耐药菌感染多见于呼吸道感染,各类器械相关感染应引起重视,应采取针对性措施。

【Abstract】 Objective To understand the epidemic status of multi-drug resistant organisms and the influence on the length of stay and hospitalization costs of nosocomial infection with multi-drug resistant bacteria. Methods Through the retrospective survey method,the infection site with MDROs and the proportion of MDROs infections from 2013 to 2015 were analyzed; the length of stay and hospitalization costs were observed and compared between the case group and control group. Results The main infection sites of MDROs infections were respiratory system,hematological system and the device-related infections,and the constituent ratios of these three sites was 50. 91% 、12. 21% and 11. 43% respectively. The proportion of MDROs infections among 3 years was 30. 2%,and the proportion were on a declining curve. The median of length of stay extended for 20 d because of MDROs infections,and the median of hospitalization costs increased about89822. 66 yuan because of MDROs infection. Conclusion There is a high proportion of MDROs infection,and the main infection sites are respiratory tract. We should pay attention to all kinds of device-related infections and it is necessary to take effective measures.
