溶质载体家族2促进葡萄糖转运蛋白成员1 HaeⅢ基因多态性与糖尿病肾病的相关性研究

【摘要】 目的探讨溶质载体家族2促进葡萄糖转运蛋白成员1 HaeⅢ(SLC2A1 HaeⅢ,g20882C>T)基因多态性与DN的关系。方法将380例T2DM患者分为DN组与T2DM组,采用PCRRLFP检测SLC2A1 HaeⅢ(g20882C>T)基因分型,探讨不同基因型患者与DN发病及相关指标的关系。结果 T2DM组与DN组CC、TT基因型比较,差异有统计学意义(P<0.05),C、T等位基因频率比较,差异有统计学意义(P<0.05)。TT基因型患者胱抑素C(Cys-C)、Hcy、UAER低于CT、CC基因型患者(P<0.05),而eGFR高于CC、CT型患者(P<0.05)。CT基因型患者Cys-C、Hcy低于CC型患者(U=5.67、4.22,P<0.05),而eGFR高于CC型患者(U=4.31,P<0.05)。Logistic回归分析结果显示,CC基因型(OR=2.105,95%CI:1.928~8.117,P<0.05)及C等位基因频率(OR=1.832,95%CI:1.003~5.462,P<0.05)为DN的危险因素。结论 SLC2A1 HaeⅢ(g20882C>T)为DN易感基因,导致Hcy、Cys-C过表达,引起DN的发生发展。

【Abstract】 Objective To investigate the relationship between Hae SLC2A1(g20882C>T) gene polymorphism and type 2 diabetic nephropathy(DN).Methods 380 cases of T2 DM were divided into DN group and non-DN group according to the levels of urinary microalbumin.Hae SLC2A1 type Ⅲ(g20882C>T) genotype was detected by PCR-RLFP.The relationship between different genotypes and the incidence of DN and related indicators were discussed.Results The frequency of CC and TT genotype had significant difference between non-DN group and DN group(P<0.05).C and T allele frequency also had significant differences(P<0.05).The levels of Cys-C[(1.54±0.49) mg/L],Hcy[(10.34±5.49) μmol/L]and UAER[(176.33 ± 34.56) μg/min]in patients with TT genotype was significantly lower than those with CT and CC genotype[(2.78±0.57) vs(3.69±0.54)mg/L,(14.68±7.54) vs(18.11±6.21)μmol/L,(246.47 ± 39.08) vs(277.83±31.72)μg/min,U=6.22,14.35,4.31,7.60,6.12 and 8.03,respectively,all P<0.05],but GFR was significantly higher than CC and CT genotype(17=4.71,6.23,P<0.05).CT genotype had lower levels of Cys-C and Hcy than CC genotype[104(32,157) vs 69(24,101)vsa 47(25,98)ml/(min·1.73 m~2),U = 5.67,4.22,P<0.05],whereas GFR higher than CC genotype(U=4.31,P<0.05).Logistic regression analysis showed that CC genotype(OR=2.105,95%CI:1.928~8.117,P<0.05) and C allele(OR=1.832,95%CI:1.003~5.462,P<0.05)were risk factors of DN.Conclusion Hae SLC2A1 Ⅲ(g20882C>T) is a susceptibility gene of DN which may result in the development of DN by promoting the overexpression of Hcy and Cys-C.
