肝脏神经内分泌肿瘤与肝细胞癌超声特征的比较研究

【摘要】 目的比较肝脏神经内分泌肿瘤(HNEN)与肝细胞癌(HCC)的超声影像学特征差异。方法回顾性分析并比较50例HNEN患者共54个病灶和同时期随机筛选的54例HCC患者共54个病灶的常规超声及超声造影特点差异。结果常规超声上,HNEN与HCC两者肝内总病灶数、边界、回声水平、回声均匀性、周边声晕及后方回声衰减的差异有统计学意义(P<0.001、0.016、<0.001、0.029、0.005、0.031),与HCC相比,HNEN主要表现为多发高回声病灶,更多病灶表现为边界不清、周边不伴声晕而伴后方回声衰减。超声造影上,HNEN比HCC更早消退为等、低增强[(32.1±17.0)s vs(39.5±12.1)s,P<0.001,(56.3±52.6)s vs(96.4±66.8)s,P<0.001)],HNEN中有58.3%(28/48)的病灶于30s内消退为等增强,75.5%(37/49)于60s内消退为低增强,HNEN比HCC有更多病灶可见包膜增强(10/54vs 3/54,P=0.038)。结论常规超声及超声造影可为HNEN与HCC的鉴别诊断提供有价值的依据。

【Abstract】 Objective To compare the features between hepatic neuroendocrine neoplasm(HNEN)and hepatocellular carcinoma(HCC)on baseline ultrasound(BUS)and contrast-enhanced ultrasound(CEUS).Methods Investigated the distinctions of imaging features on BUS and CEUS between 50 HNEN patients(54 lesions)and 54 HCC patients(54 lesions).Results On BUS,distinctions of the total number of lesions located in liver,margin,level and uniformity of echogenicity,acoustic halo and posterior echo attenuation between HNEN and HCC were statistically significant(P<0.001,0.016,<0.001,0.029,0.005,0.031).Compared with HCC,HNEN manifested as multiple hyper-echoic lesions,and more lesions characterized by vague margin,absent of acoustic halo while accompanied with posterior echo attenuation.On CEUS,HNEN subsided into iso-enhancement or hypo-enhancement earlier than HCC[(32.1±17.0)s vs(39.5±12.1)s,P<0.001,(56.3±52.6)s vs(96.4±66.8)s,P<0.001].58.3%(28/48)HNEN subsided into iso-enhancement within 30 s,and 75.5%(37/49)HNEN subsided into hypo-enhancement within60 s.More HNEN presented capsule enhancement in the delay phase than HCC(10/54 vs 3/54,P=0.038).Conclusions BUS and CEUS can provide some valuable information for differential diagnosis between HNEN and HCC.
