【摘要】 目的探讨超声双重造影(DCEU)对大肠肿瘤筛查的临床应用价值。方法 (1)受检查者先行常规经腹壁超声检查(CTU),然后清洁肠道,将胃肠声学造影剂从肛门灌入肠道后,再进行大肠充盈型造影超声检查(IFCU)。(2)对怀疑大肠肿瘤者进行静脉造影超声检查(ICEU)、纤维肠镜及病理学检查。(3)DCEU、纤维肠镜、病理学检查三者结果进行对比分析,经统计学处理后,研究DCEU对大肠肿瘤筛查的临床应用价值。结果 (1)CTU及IFCU检查266例,DCEU检查45例;经病理学检查确诊大肠肿瘤41例,CTU发现20例,DCEU发现39例,表明DCEU对大肠肿瘤的筛查更具检出性(χ2=21.81,P=0.00)。(2)大肠肿瘤DCEU声像图各具特点。(3)DCEU对大肠肿瘤定位准确率较CTU明显增高(χ2=28.15,P=0.00)。(4)对良、恶性大肠肿瘤定性符合率DCEU为88.89%、96.88%,纤维肠镜为100%、93.75%,DCEU及纤维肠镜对大肠肿瘤的检出均是有效的(P>0.05)。结论大肠双重超声造影检查法为大肠肿瘤好发人群筛查提供有效可行的手段,具有重要的临床推广应用价值。更多还原

【Abstract】 Objective To investigate the value of clinical application of double contrast ultrasonography(DCEU)in screening of large bowel tumours.Methods(1)Conventional transabdominal ultrasonography(CTU)was inspected firstly,and then clean the intestines.The gastrointestinal acoustic contrast agent was poured into the intestine from the anus,then the intestinal filling contrast ultrasonography(IFCU)was carried out.(2)The patients of large bowel tumours suspected was undergo intravenous contrast enhanced ultrasonography(ICEU)and fiber colonoscopy and pathological examination.(3)The results of DCEU and fiber colonoscopy and pathological examination were compared and analyzed,to investigate the value of clinical application of DCEU in screening of large bowel tumours after statistical treatment.Results(1)There were 266 cases were performed CTU and IFCU,and 45 cases for DCEU.41 cases of large bowel tumours diagnosed by pathological examination,20 cases were revealed by CTU,39 cases were revealed by DCEU,indicates that the screening for large bowel tumours by DCEU was more positive(χ2=21.81,P=0.00).(2)The sonographic characteristics of large bowel tumours by DCEU were different.(3)The accurate rate of DCEU for large bowel tumours was significantly higher than that of CTU(χ2=28.15,P=0.00).(4)The qualitative coincidence rate of benign and malignant colorectal tumors of DCEU was 88.89%and 96.88%,fiber colonoscopy was 100%,93.75%.It showed that the two methods was effective for the detection of large bowel lesions(P>0.05).Conclusions Double contrast ultrasonography provides an effective and feasible method for the screening of large bowel tumours,and has important clinical application value.更多还原
