微血管成像与超声造影在鉴别甲状腺结节良恶性的价值研究

【摘要】 目的对比微血管成像(SMI)与超声造影(CEUS)两种技术在甲状腺结节鉴别诊断中的应用价值。方法对57例共76个甲状腺结节分别进行SMI和CEUS检查,观察结节血流分布模式和CEUS增强模式,评价两者在鉴别诊断甲状腺结节良恶性中的价值。结果76个甲状腺结节中恶性结节41个(53.9%),良性结节35个(46.1%)。CEUS诊断为恶性结节39个(51.3%),良性结节37个(48.7%);SMI诊断为恶性结节42个(55.3%),良性结节34个(44.7%);CDFI诊断为恶性结节34个(44.7%),良性结节42个(55.3%)。SMI与CEUS相比,其诊断的特异度、灵敏度、准确度、阳性预测值及阴性预测值均无统计学差异(P>0.05);SMI技术较CDFI有更高的灵敏度及准确度(P<0.05),特异度、阳性预测值和阴性预测值差异则无统计学意义(P>0.05)。结论SMI技术及CEUS增强模式对甲状腺结节良恶性的鉴别诊断效能类似。

【Abstract】 Objective To compare the value of superb micro-vascular imaging(SMI)and contrast-enhanced ultrasound(CEUS)in the diagnosis of benign and malignant thyroid nodules.Methods Totally 76 thyroid nodules in 57 patients were examined by SMI and CEUS,and then compare the diagnostic value of the two methods.Results Among the 76 nodules,there were 41 malignant nodules(53.9%)and 35 benign nodules(46.1%).39(51.3%)were diagnosed as malignant and 37(48.7%)were benign by CEUS 42(55.3%)were diagnosed as malignant and 34(48.7%)were benign.By SMI 34(44.7%)were malignant and 42(55.3%)were benign by CDFI.There were no differences between SMI and CEUS in terms of sensitivity,specificity,acc-uracy,positive predictive value and negative predictive value(P>0.05 for all).SMI was better than CDFI in sensitivity and accuracy.But there were no differences in specificity,positive predictive value and negative predictive value between the two methods(P>0.05).ConclusionsThe diagnostic efficacy of SMI is similar to that of CEUS enhancement patterns.
