组织二尖瓣环位移评价冠状动脉慢血流患者左心室收缩功能

【摘要】 目的采用超声组织二尖瓣环位移技术(TMAD),评价冠状动脉慢血流患者(SCF)的左心室收缩功能,探讨其临床应用价值。方法临床诊断为SCF的患者61例(SCF组),采用超声心动图检测左心室射血分数(LVEF)、二尖瓣环室间隔位点位移(TMAD1)、侧壁位点位移(TMAD2)、平均位移(TMAD Midpt)和平均位移率(TMAD Midpt%),并与60例健康成年人(对照组)比较。结果 (1)与对照组比较,SCF组TMAD1、TMAD2、TMAD Midpt和TMAD Midpt%均减低(P<0.05),但LVEF无显著变化;(2)TMAD各参数均与LVEF呈正相关(r=0.20、0.26、0.29、0.24,P<0.05);(3)TMAD各参数与年龄、心率无相关(P>0.05),TMAD1、TMAD2、TMAD Midpt与左室舒张末容积(LVEDV)呈正相关(r=0.19、0.20、0.25,P<0.05),而TMAD Midpt%与LVEDV无相关(P>0.05)。结论冠状动脉慢血流患者左心室收缩功能减低。TMAD参数中TMAD Midpt%不受左心室容积影响,是可简便、准确、灵敏评价左心室收缩功能的新指标。

【Abstract】 Objective To explore the value of tissue mitral annular displacement(TMAD)in evaluating left ventricular systolic function in patients with slow coronary flow(SCF).Methods Sixty-one patients with SCF diagnosed by coronary angiography were enrolled in this study,sixty healthy subjects served as control group.The left ventricular ejection fraction(LVEF),mitral annular displacement at interventricular septum(TMADl),lateral wall(TMAD2),average displacement(TMAD Midpt),and the ratio of average displacement(TMAD Midpt%)were measured by echocardiography.Results(1)TMAD1,TMAD2,TMAD Midpt and TMAD Midpt% were lower in patients with SCF than those in control group(P<0.05),but LVEF showed no significant difference(P>0.05).(2)TMAD parameters all correlated positively with LVEF(r=0.20,0.26,0.29,0.24,P<0.05).(3)TMAD parameters had no correlation with age and heart rate(P>0.05).And except for TMAD Midpt%,TMAD parameters also correlated positively with left ventricular end diastolic volume(LVEDV)(P<0.05).Conclusions Left ventricular systolic function was impaired in patients with SCF.Among TMAD parameters,TMAD Midpt%is not affected by LVEDV and could provide a simple,rapid and sensitive approach to evaluate left ventricular systolic function.
