STAT3基因多态性与子宫内膜异位症合并不孕GnRH-a治疗效果的关系
Study on the relationship between STAT3 gene polymorphism and the curative effect of GnRH-a in treatment of endometriosis combined with infertility
【摘要】 目的探讨信号转导和转录活化因子3(STAT3)基因多态性与子宫内膜异位症(EMs)合并不孕性腺激素释放激素激动剂(GnRH-a)治疗效果的关系。方法选择经腹腔镜手术病理证实的EMs合并不孕患者60例,进行STAT3基因多态性检测,确定STAT3基因分型,比较不同基因类型治疗后妊娠成功率、复发率及治疗前后痛经评分、CA125水平。结果 1EMs合并不孕患者rs2293152为STAT3基因多态性位点,3种基因型分布分别为:CC纯合型18例(30.00%),CG杂合型24例(40.00%),GG纯合型18例(30.00%);2CC纯合型、CG杂合型、GG纯合型患者治疗12个月内妊娠成功率分别为72.22%、50.00%、33.33%,复发率分别为0、8.33%、11.11%,差异有统计学意义(χ~2妊娠=26.852,P妊娠<0.05;χ~2复发=5.632,P复发<0.05);3治疗6个月、12个月痛经评分、CA125水平顺位:CC纯合型<CG杂合型<GG纯合型,组间比较差异有统计学意义(P<0.05)。结论 STAT3基因多态性对EMs合并不孕GnRH-a治疗效果有一定影响,携带G等位基因的基因型患者治疗效果相对较差。
【Abstract】 Objective To explore the relationship between signal transducer and activator of transcription 3( STAT3) gene polymorphism and curative effect of gonadotropin releasing hormone agonist( GnRH- a) in treatment of endometriosis combined with infertility.Methods Sixty infertile patients with endometriosis confirmed by laparoscopic surgery and pathological examination were selected,then STAT3 gene polymorphism was detected to determine STAT3 genotypes.Pregnancy successful rates and recurrence rates after treatment,dysmenorrhea scores and CA125 levels before and after treatment were compared among different genotypes.Results Rs2293152 was the polymorphism site of STAT3 gene in the patients with endometriosis combined with infertility,the three genotypes were CC homozygous type( 18 cases,30.00%),GG heterozygous type( 24 cases,40.00%) and GG homozygous type( 18 cases,30.00%).The successful rates of pregnancy in CC homozygous type,CG heterozygous type and GG homozygous type patients were 72.22%,50.00% and 33.33% within 12 months after treatment,respectively; the recurrence rates were 0.00%,8.33% and 11.11%,respectively,there were statistically significant differences( χ~2= 26.852,P<0.05; χ~2= 5.632,P<0.05).The rank of dysmenorrheal scores and CA125 levels after 6 and 12 months of treatment: CC homozygous type<GG heterozygous type<GG homozygous type,there were statistically significant differences between different groups( P<0.05).Conclusion STAT3 gene polymorphism has a certain effect on the curative effect of GnRH-a in treatment of endometriosis combined with infertility.The patients with G allele has a relatively poor therapeutic effect.
